Cytotechnologist-Attended On-Site Evaluation of Adequacy for Metastatic Disease Involving Bone and Soft Tissue

Abstract: Introduction: Image-guided fine needle aspiration (FNA) of the musculoskeletal system is expensive to perform and repeat, so on-site evaluation of adequacy (OSEA) is increasingly used to ensure an optimal sample. Metastatic disease to the musculoskeletal system is not uncommon and often requires OSEA when sampled. At large academic centers, cytotechnologists have filled the increased demand for OSEA. However, the performance of cytotechnologists has not been compared with that of cytopathologists for OSEA in F… Show more

“…Cytotechnologists do not currently participate in OSEA of liver lesions at our institution although they are an integral part of OSEA for other body sites [8,11,12,13,14]. Our data offer compelling evidence that OSEA of liver lesions by cytotechnologists may indeed be possible by showing the noninferiority of cytotechnologists to cytopathologists in this role.…”

“…The cost is justified easily on an institutional scale when the decreased rebiopsy rate and efficient use of a cytopathologist's time are considered [17], but few medical delivery systems are integrated well enough to allocate resources so holistically. As such, the practice remains the domain of large academic centers [11,12,13,14,18]. This is likely to change, given the emergence of telecytology and the growth of accountable care organizations.…”

“…the screening of nongynecological specimens, on-site evaluation of adequacy [1,2,3] and interfacing cytopathology with molecular methods [4]. Regulatory practices require quality performance measures for evaluating an individual's skill at interpreting Pap tests [5,6,7].…”

“…For instance, CTs have demonstrated usefulness in the performance of on-site evaluation of adequacy (OSEA; also known as Rapid On-site Evaluation or ROSE) [1,2,3,4], the screening of nongynecological specimens [5] and interfacing cytopathology with molecular methods [6]. Concurrently, thyroid fine-needle aspirations (FNA) have more than septupled at our institution over the past decade [3] and are becoming the most common nongynecological specimens in most cytopathology laboratories.…”
