Dacryolithiasis: A Review

Abstract: The management of dacryolithiasis is driven by the goal of resolution of secondary obstruction and/or inflammation. Although a large number of dacryoliths are incidentally found during dacryocystorhinostomy, certain clinical features such as unilateral sac distension, particularly those with a palpable firm medial canthal mass, might lead one to have a high index of suspicion. It remains unclear if the incidental finding of a dacryolith during a dacryocystorhinostomy has a favorable prognostic value.

“…Actinomyces are normal commensal bacteria in humans and primarily cause opportunistic infections during immunosuppressive state or when loss of continuity of epithelial lining in mucosa occurs. [ 3 12 14 ] In our case, there seems to be no certain predisposing factor for Actinomyces infection to occur. In our case, it is difficult to establish conclusively if the canalicular infection had any role to play in the development of nasolacrimal duct obstruction (NLDO).…”

“…Some authors have described the components of dacryoliths to reveal the mechanism of their formation. Several studies have described the etiology and gender and age-association of nasolacrimal duct obstruction (NLDO) and smoking; however, the results of each study differed in accordance with limited attendance [1,2] .…”

“…The goal of dacryolithiasis management is to resolve secondary obstruction and inflammation by removing the dacryolith, and Ext-DCR is the gold standard for treating NLDO caused by dacryolithiasis. 2 EN-DCR may be another option for removing a dacryolith and reestablishing lacrimal drainage without requiring a skin incision. 4 Before 2009, Ext-DCR was the most commonly used DCR technique in our clinic; however, once the diode laser device became available, TCL-DCR became our patients’ preferred surgery as a minimally invasive technique with a lower risk of bleeding and skin scarring.…”
