Death by Suicide in Graves' Disease and Graves' Orbitopathy: A Nationwide Danish Register Study

Abstract: Mortality by suicide was increased in Graves' disease overall, most significantly in patients with GO, also after adjustment for pre-existing somatic and psychiatric disease. These findings indicate that GD and GO may have a significant role in the pathophysiological mechanisms of suicidal behavior. Beyond independent confirmation, reasons for this need to be explored in order to introduce preventive measures.

“…First, autoimmune thyroid disease itself increases the risk of suicide in general population. For example, a recent large cohort study in the Danish population showed that the suicide mortality was increased in patients with Graves' disease, most significantly among patients with Graves' orbitopathy, even after adjusting for pre-existing somatic and psychiatric disease 54 . Another study of 111,565 people with Hashimoto's disease in Danish population also showed the increased suicide mortality 55 .…”

“…Graves' orbitopathy (GO) is an orbital disorder closely associated with autoimmune thyroid disease [1]. GO is responsible for considerable psychosocial morbidity, personal and societal cost [2,3], can rarely cause blindness [4], and is associated with increased mortality including that from suicide [5,6]. Most patients present with bilateral disease though asymmetric or unilateral disease is also observed in a minority of cases.…”

“…3 The burden of morbidity is considerable, with a marked impact on quality of life 4 and mental health, 5 which may lead to increased rates of suicide. 6 Over 90% of patients with TED also have hyperthyroidism, 3 however, patients may be hypothyroid or euthyroid. 7 The manifestations of TED typically involve expansion of soft tissues within the bony orbital cavity, with associated ocular inflammation.…”
