Decreased photoreceptor inner segment/outer segment junction reflectivity in patients with idiopathic epimacular membrane

Abstract: Purpose To assess the effects of idiopathic epimacular membrane (IEMM) on retinal pigment epithelium (RPE), photoreceptor inner segment/outer segment (IS/OS) junction, and external limiting membrane (ELM) reflectivities to determine functional alterations in these layers using optical coherence tomography (OCT) image analysis. Methods Fifty eyes of 50 patients with untreated IEMM and 41 eyes of 41 age-and sex-matched healthy controls with normal OCT scans were retrospectively reviewed. A single masked physicia… Show more

“…Hood et al [20] showed that decreased cone function was correlated with the intensity of the inner segment ellipsoid (ISe) band. With this, Yılmaz Tugan et al [21] and Toprak et al [22] determined that the reflectivities of the EZ (formerly known as IS/OS) may be related to prognosis in patients with macular hole and epiretinal membrane. For these reasons, the reflectivities of these structures reflect their functions.…”

“…with ectopic inner foveal layers) with, respectively, one and one error for the SS-OCT and three and five errors for the PCI device (p = 0.02) (Figs 2 and 3 ). These findings resulted in a clinically relevant difference in terms of refractive PE, that would have brought to a significantly greater mean myopic shift of 0.69 D with the PCI compared to 0.24 D for the SS-OCT device (p<0.05).The cause of AL measurement errors in patients with severe ERM could be related to: (1) a thick ERM / posterior hyaloid (that being highly reflective accentuate the vitreo-retinal interface), (2) the attenuation of the RPE reflectivity caused by the ERM itself, and (3) by a thicker retina and the presence of ectopic inner foveal layers [ 8 , 25 ].…”

“…Reflectance profiles were taken in an automated fashion using a customized script including plot profile extraction in Fiji. This approach has been described elsewhere as a robust method to access EZ integrity [ 11 , 19 , 20 ]. Manual adjustment was not applied.…”

“…However, the long-term outcome of EZR during anti-VEGF treatment remains unknown. Additionally, in several studies, the quantitative analysis of EZR was laborious and time consuming as the measurements were manually obtained and mostly at a limited number of regions of interest [ 16 , 19 , 20 ]. The aim of this study is to evaluate the ellipsoid zone outcome (EZR and EZ-RPE distance) during a 3- to 5-year follow-up of DME patients under anti-VEGF therapy and to find potential correlation with the visual acuity outcome beyond 1 year after treatment initiation.…”
