Deep vein thrombosis in spinal cord injury: effect of prophylaxis with calf compression, aspirin, and dipyridamole

Abstract: Abstract. Deep Vein thrombosis is very common in spinal cord injury patients, and a randomized study comparing the prophylactic use of external pneumatic calf compression, aspirin and dipyridamole has been carried out.Key words: Deep vein thrombosis; Spinal injury; Platelets; Factor VIII.DEEP vein thrombosis (DVT) occurs frequently in patients with a history of recent trauma or surgery, especially when followed by prolonged immobilisation (Sevitt & Gallagher, 1961;Sevitt, 1969). Therefore, it would be anticip… Show more

“…Nineteen other trials enrolled diverse groups of moderately to severely injured patients, including eight trials in patients with spinal cord injury 30,[166][167][168][169][170][171][172] and four studies in patients with orthopedic injuries. [173][174][175][176] Studies evaluated both mechanical (eg, IPC, myostimulation, continuous passive motion) and pharmacologic (eg, LDUH, LMWH) interventions, but no randomized trials examined IVC fi lter placement or use of surveillance ultrasound.…”

“…1,2 Depending on diagnostic methods used, rates of deep venous thrombosis (DVT) are reported between 10 and 50% in this specific patient population. [3][4][5][6][7] DVTs were detected in 20% of patients admitted to rehabilitation facilities. 8,9 Predictors for the development of DVT are male sex, length of hospitalization, comorbidities, and severity of injuries.…”

“…Outcomes have been better when treatment protocols used a combination of measures that targeted various risk factors individually. 1,2 Di erent protocols using oral anticoagulants, heparin, low molecular weight heparins, vena caval ®lters and antithromboembolic stockings have been demonstrated to provide e ective prophylaxis in patients with spinal cord injuries. 3 Low molecular weight heparins (LMWH) were introduced in the later half of the eighties.…”
