Abstract: BackgroundPhysicians routinely encounter diagnostic uncertainty in practice. Despite its impact on health care utilization, costs and error, measurement of diagnostic uncertainty is poorly understood. “…The presence or absence of diagnostic uncertainty was assessed through a 6‐point Likert scale ranging from strongly agree to strongly disagree. Based on our recent work, we used an operational definition of diagnostic uncertainty: “a subjective perception experienced by the clinician that reflects their inability to provide an accurate explanation for the patient's health problem.” The operational definition and the presence or absence of documentation elements for direct expression and indirect inference were useful in assessing whether diagnostic uncertainty was present. We focused on documentation at the end of visit (assessment; plan) as the reference point for measurement because additional diagnostic data that emerges during the visit often helps reduce uncertainty at the beginning.…”
