“…Global scar burden has been shown to be inversely proportional to LV reverse remodeling amongst both ischemics [41], non-ischemics [42], and mixed populations [4345]. Functional improvement [41,42,46] and survival [47] are also inversely proportional to scar burden. Retrospective analysis has confirmed that favorable markers for response include smaller scar size and fewer areas of transmural scar [45].…”

“…46,47 Those patients with transmural necrosis in either the septal or inferolateral walls experience an absence of improvement in LV volumes at 6 months. Performance of LGE-CMR prior to CRT implantation can help to guide the procedure; therefore, the lead can be delivered to viable tissue targets, improving event-free survival.…”

“…Presence of scar in the posterior-lateral LV, the region typically targeted for LV lead placement, was shown to reduce rates of clinical response and reverse remodelling of the LV in several trials. [29][30][31][32] An LV lead with its tip in such an area of scar is less likely to effectively pace the LV. 33 Despite these findings, pre-implant cardiac MRI remains uncommon practice due to expense, inconvenience and inability to perform in patients with an existing pacemaker or defibrillator.…”
