A 70-year-old female patient presented with acute severe respiratory distress at a district general hospital. Medical history included type 2 diabetes, recurrent pulmonary embolisms and pre-existing diaphragmatic hernia containing part of the liver. Despite initial treatment with steroid inhalers, her clinical picture rapidly deteriorated requiring emergency intubation and positive pressure ventilation. Imaging investigations revealed tension enterothorax and hepatothorax with tracheal deviation. The patient was transferred and underwent an emergency laparotomy at the Regional Oesophagogastric Unit. A large diaphragmatic hernia (central tendon defect) which contained the duodenum, porta hepatis, right lobe of liver, gallbladder and right colon was reduced and successfully repaired. Her postoperative course was uneventful with no signs of recurrence at 2 months follow-up.This case describes an extremely rare and life-threatening condition of tension enterothorax and hepatothorax, which should be considered in the differential diagnosis of acute respiratory distress with tracheal deviation.

Traumatic tension enterothorax in New Zealand: a case report

Traumatic rupture of the right hemidiaphragm occurred following a high-velocity motor vehicle collision in the case presented herein. The resulting herniation of small bowel loops into the thorax resulted in hemodynamic and respiratory compromise due to pressure effects on the right heart and major vessels. The patient's hemodynamic status improved with reduction of enterothorax, and the diaphragmatic defect was repaired. We discuss the available literature and learning points from this rare case.

Edi t ö r ' e M ek t u pBizde, benzer bir şekilde, kliniğimize solunum sıkıntısı ve karın ağrısı şikayetleri ile gelen ve hikayesinde 18 yıl önce araç dışı trafik kazası öyküsü olan hasta ile ilgili deneyimimizi paylaşmak istedik. Acil servise karın ağrısı ve solunum sıkıntısı ile başvuran 42 yaşındaki erkek hastanın, 2 gün önce başlayan karın ağrısı ve son zamanlarda artış gösteren solunum sıkıntısı mevcuttu. Karın ağrısı, göbek çevresinden başlayıp sırta doğru yayılım gösteren kuşak tarzında idi. Özgeçmişinde, 10 yıl boyunca günde 1 paket sigara kullanım öyküsü ve 1996 yılında geçirdiği araç dışı trafik kazası mevcuttu. Bu travma nedeniyle aynı dönemde vertebra stabilizasyon operasyonu geçirmişti. Hastanın yine aynı travmaya bağlı hemiplejisi mevcuttu. Soy geçmişinde özellik yoktu. Yapılan biyokimyasal incelemede; amilaz yüksekliği (2209 U/L), karaciğer enzimleri yüksekliği (AST:135 U/L, ALP:134 U/L, GGT:1238 U/L), ve hiperbilirubinemi (Total Bilirubin:4,6 mg/dL) saptandı. Hastanın direk posteroanterior akciğer grafisinde akciğerin yukarı doğru itildiği ve sağ hemitoraksta barsak anslarıyla uyumlu görünüm izlendi. Bununla beraber omurgalardaki deformite de görüldü. Ayakta direk karın grafisi sağ diyafragmanın durumu dışında normaldi. Karın ağrısına yönelik yapılan ultrasonunda (USG) safra kesesi içerisinde çok sayıda milimetrik taş ve intrahepatik safra yollarında minimal genişleme saptandı. Ranson skoru sıfır olan hasta servise yatırıldı. Oral alımı kesilerek intravenöz hidrasyon sağlandı. Dispnesi olan hastanın takipnesi de mevcuttu. Yapılan abdominopelvik bilgisayarlı tomografide, safra kesesinde izlenen taşlara ek olarak sağ hemidiyafragmadaki geniş defektten (yaklaşık 15 cm) tüm ince barsak ansları, hepatik fleksura, çıkan kolon, çe-kum, süperior mezenterik arter ve dallarının sağ hemitoraksa herniye olduğu ve barsak anslarının sağ apex komşuluğuna kadar ilerlediği ayrıca duodenumun 2. kısmının orta hatta itildiği izlendi (Resim-1). Hastanın, akut pankreatit tanısıyla servisteki yatışının 3. gününde amilaz değerleri geriledi ve bilirubin seviyeleri normale döndü. Hastaya kolesistektomi ile beraber diyafragma herni onarımı planlanarak ameliyata alındı. Hastaya orta hat kesisi ile laparotomi yapıldı. Gözlemde, sağ diyafragmadaki 20 cm'lik defektten tüm ince barsakların, çıkan kolonun, çekumun, süperior mezenterik arter ve dallarının hemitoraksa fıtıklaştığı izlendi.
