Background: With the evolution of the endovascular devices, the management of endovascular interventions has become the current standard therapy for traumatic carotid-cavernous fistula (TCCF). However, only endovascular treatment may not be feasible in some patients with atypical TCCF. Case Description: We described three complex TCCFs that could not be managed by conventional endovascular methods. The first patient had recurrent TCCF previously treated by muscle embolization and ligation of affected carotid arteries 23 years ago. Another two patients had TCCFs association with large pseudoaneurysm within the sphenoid sinus. In each patient, the fistula was successfully closed by trapping procedure using a combination of endovascular and surgical treatment. Conclusion: To reduce costs of treatment, trapping operation by combining surgical and endovascular treatment may be considered as an alternative option for complex TCCF which has some features including chronic stage, preexisting carotid artery ligation, or association with large venous pouch of the cavernous sinus or sphenoid sinus pseudoaneurysm.

 Embolizasyon ile tedavi edilebilen nöro-vasküler patolojiler Nörovasküler patolojilerde temel embolizasyon yöntemleri ve dikkat edilecek hususlarVasküler yolla embolizasyon, arteriyel ve/ veya venöz yolu kullanarak kanama gibi durumlarda normal vasküler yapıların, anevrizma veya AVM gibi vasküler patolojilerde veya vasküler tümörlerde olduğu gibi patolojik vasküler yapıların farklı embolizan maddeler ile geçici veya kalıcı tıkanmasıdır.Embolizasyon işlemine başlamadan önce patolojik yapı mevcut görüntüleme yöntem-leri ile iyice değerlendirilmeli, yapılacak tedavi şekli ve alternatifleri ile birlikte kullanılabilecek malzemeler belirlenmeli, işlem sırasında oluşabilecek vazospazm, diseksiyon gibi komplikasyonlar için hazırlıklı olmalı ve bu komplikasyonların yönetimi için ilaç ve malzeme temini yapılmalıdır. İşlemde kullanılacak koil veya sıvı embolizan ajan gibi maddeler ile kateter-mikrokateter uygunluğu değerlendirilmelidir. Sıvı karışım ile genişle-yebilen embolizan maddeler ile veya distalde daralan mikrokateter kullanıldığında koiller ile katater tıkanması görülebilir. Aksine geniş çapı olan kateter içinden küçük çaplı koil atarken koil katlanabilir. Eğer çift mikrokateter kullanılacaksa buna uygun iç çapı olan guiding kateter seçilmelidir. Özellikle tortuoz damarlarda uzun sheet ve DAC gibi destek sağlayan malzeme kullanımı gerekebilir.Embolizasyon geçici veya kalıcı olarak yapılabilir. Geçici karotis oklüzyonu yaparak serebral beslenme, willis poligonu-kominikan sistem değerlendirmesi veya sıvı embolizan maddeler ile embolizasyon sırasında akımı yavaşlatarak veya keserek reflü veya distale migrasyonun önlenmesi ayrılamayan balonlar ile yapılan geçici embolizasyon ör-nekleridir.Kalıcı embolizasyon, embolizan maddeler ile organ veya patolojik vasküler yapıların kalıcı olarak kan akımının azaltılması veya tamamen engellenmesidir. Bu amaçla kullanılan başlıca embolizan maddeler, sıvı-sklerozan maddeler, partiküller ve mekanik oklüzyon cihazlarıdır.
