Delayed ulnar neuropathy at the wrist following open carpal tunnel release

Abstract: Open carpal tunnel release is a common and successful treatment of median neuropathy at the wrist (carpal tunnel syndrome). We report a case of delayed ulnar neuropathy at the wrist with onset 2 months after open carpal tunnel release. Clinical findings, electrophysiological studies, magnetic resonance imaging, and surgical exploration demonstrated ulnar nerve compression at Guyon's canal resulting from translocation of the carpal tunnel contents. To our knowledge, this is an unreported complication of open ca… Show more

“…Direct ulnar nerve injury may occur during an open carpal tunnel release because of the nerve's anatomical adjacency, but only one case of delayed ulnar neuropathy at the wrist following open carpal tunnel release has been reported,3 which demonstrated that the translocation of carpal contents resulted in the compression of the ulnar nerve in Guyon's canal several months after open carpal tunnel release. Magnetic resonance imaging of the involved wrist revealed that the superficial and deep finger flexor tendons with the carpal tunnel had been translocated over the hook of hamate and into Guyon's canal 3…”

“…Ulnar nerve injury near the wrist may be a rarely reported surgical complication. In particular, delayed ulnar neuropathy at the wrist following surgery is even more rare 3. In this paper, we report a case of delayed ulnar neuropathy at the wrist that was resolved with conservative management following an open carpal tunnel release surgery.…”

“…Carpal tunnel syndrome is the most common nerve entrapment disorder of the upper extremity with an estimated life time risk of 10% and an overall prevalence of 2.1% in the general population 11) . The syndrome is usually related to compression of the median nerve within the carpal tunnel because of a variety of etiologic factors 6) .…”

“…4) To our knowledge, only two previous reports have described delayed compression neuropathy due to the volar and ulnar displacement of the flexor tendons from the carpal tunnel. 5,6) We report an acute ulnar neuropathy due to a similar anatomical change following carpal tunnel release, which gradually improved with observation only.…”
