Introduction/Background: Suicidal behavior in delusional disorder (DD) patients has been poorly studied. Objective: Investigate the demographic, environmental, psychosocial and clinical characteristics of the suicidal behavior in a group of patients with DD. Method: Retrospective descriptive study of DD cases registered at Psychiatry and Mental Hygiene Clinic of Cordoba according to DSM-IV criteria was conducted. We obtained a sample of 261 DD patients who met the inclusion criteria, of them 26 cases presented evidence of suicidal behavior. Data and variables collected were divided into 4 groups: I. Socio-demographic and general data. II. DD risk factors (personal and family). III. DD clinical picture and diagnosis (presentation, symptoms, disability, use of health care resources, treatment, and evolution) and lethal methods used. Results: The mean age of the patients was 47 years old and 60% were males. Socio-demographic data show that 46.2% of patients with suicidal behavior lived in rural areas, only 15.4% were analphabet, 57.7% were unemployed, and 26.9% were receiving a pension because of illness or retirement. The persecutory subtype with 8 cases was the most frequent presentation, followed by grandiose (6 cases) and jealous (6 cases), mixed (3 cases), erotomanic (2 cases) and somatic (1 case) subtypes. Discussion and conclusion: It is necessary to conduct future prospective studies to investigate the protective and risks factors associated with the suicidal behavior in DD patients.

Psychosis is relatively common in later life and can present in a wide variety of contexts, including early-onset and late-onset schizophrenia, delusional disorder, mood disorders, and various dementias. It can also occur as the result of numerous medical and neurological diseases and from the use of certain medications. Although identifying the cause of psychosis in older patients can be challenging, the unique clinical features associated with the different disorders can help in making the diagnosis. Accurate diagnosis of psychosis in older populations is essential, as its treatment varies depending on the context in which it appears. Despite the safety concerns regarding the use of antipsychotics in older patients, certain pharmacological treatments appear to be both efficacious and reasonably safe in treating psychosis in older populations. Additionally, although research is limited, numerous psychosocial therapies appear promising. This review summarizes the literature on the epidemiology, clinical characteristics, neuroimaging, and treatment of psychosis in later life, and serves as an update to past reviews on this topic.

The aim of this paper is to help clinicians better understand how erotomania originates in order to facilitate treatment and make it more effective. Data sources are the narratives of six women who spoke in detail about the beginnings of their delusional beliefs and about the nature of the evidence that convinced them that their beliefs were well-founded. In every case, low self-esteem and emotional arousal preceded the emergence of the delusion. Misperceptions and misattributions appeared responsible for keeping the delusion alive. Despite external disconfirmation, social isolation protected the delusional beliefs from revision and extinction. The erotomanic delusion provided a sense of well-being that probably contributed to its maintenance. As well, a delusion-induced boost in well-being delayed help-seeking. Recommendations for treatment include staged interventions, first establishing a therapeutic alliance with a focus on understanding the psychological factors contributing to the origin and maintenance of the delusion. The next stage is the provision of social support and strategies directed at the restoration of self-esteem. The third stage is the gradual introduction of techniques to correct cognitive biases. Medication and risk management form an integral part of overall management. Objective evidence for the effectiveness of this approach is, however, not yet available.
