Aim To evaluate the effects of (a) transcrestal sinus‐floor elevation (TSFE) and (b) residual bone height on long‐term implant survival. Materials and Methods Chi‐squared and t tests were used for descriptive comparison of the groups. Kaplan–Meier survival curves and corresponding log‐rank tests were used to investigate implant survival over time. Multivariable Cox regressions were performed for the total population and experimental group. Results A total of 634 patients received 648 implants with TSFE, while 674 implants without TSFE served as controls. Thirty implant failures occurred in the experimental group and 28 in the control group. Ten‐year Kaplan–Meier survival curves for the 157 implants (24.3%) still under observation showed a probability of survival of 93.7% for the implants with TSFE and 92.9% for the 72 implants without TSFE (p = .678). The probability of 10‐year survival of all implants in the experimental group decreased to 77.4% for implants placed in residual bone heights of 1–3 mm, compared with 95.7% for implant sites with bone heights of 4–6 mm and 97.6% for bone heights of >6 mm. Conclusions Transcrestal sinus‐floor elevation has no negative effect on the long‐term implant survival. Membrane perforation or negligible bone height, however, reduces the probability of 10‐year survival.

Synthetic osteo‐promoting materials that are able to stimulate and accelerate bone formation without the addition of exogenous cells or growth factors represent a major opportunity for an aging world population. A co‐assembling system that integrates hyaluronic acid tyramine (HA‐Tyr), bioactive peptide amphiphiles (GHK‐Cu2+), and Laponite (Lap) to engineer hydrogels with physical, mechanical, and biomolecular signals that can be tuned to enhance bone regeneration is reported. The central design element of the multicomponent hydrogels is the integration of self‐assembly and enzyme‐mediated oxidative coupling to optimize structure and mechanical properties in combination with the incorporation of an osteo‐ and angio‐promoting segments to facilitate signaling. Spectroscopic techniques are used to confirm the interplay of orthogonal covalent and supramolecular interactions in multicomponent hydrogel formation. Furthermore, physico‐mechanical characterizations reveal that the multicomponent hydrogels exhibit improved compressive strength, stress relaxation profile, low swelling ratio, and retarded enzymatic degradation compared to the single component hydrogels. Applicability is validated in vitro using human mesenchymal stem cells and human umbilical vein endothelial cells, and in vivo using a rabbit maxillary sinus floor reconstruction model. Animals treated with the HA‐Tyr‐HA‐Tyr‐GHK‐Cu2+ hydrogels exhibit significantly enhanced bone formation relative to controls including the commercially available Bio‐Oss.

Posterior maksillada yetersiz kemik yüksekliği durumunda, implant yerleştirilmesi için sinüs ogmentasyonu tekniği uzun yıllardır başarılı bir şekilde uygulanmaktadır. Bu çalışmada; sinüs tabanı altında farklı kemik yüksekliklerinde sinüs ogmentasyonu ile eş zamanlı yerleştirilen implantların sağkalım oranlarının değerlendirilmesi amaçlanmıştır. Çalışmada primer belirleyici değişken sinüs tabanının altında kalan kemik yüksekliği, primer sonuç ise implantların sağkalım oranlarını değerlendirmektir. Gereç ve Yöntemler: Sinüs ogmentasyonu ile eş zamanlı olarak implant yerleştirilen 64 (28 erkek, 36 kadın) hasta bu retrospektif çalışmaya dâhil edildi. Hastalar, sinüs tabanı altındaki kemik yüksekliğine göre Grup 1 (<5 mm) ve Grup 2 (≥5 mm) olmak üzere iki gruba ayrıldı. Kemik yüksekliğinin değerlendirilmesinde konik ışınlı bilgisayarlı tomografi kullanıldı ve ölçümler yapıldı. İmplantların markaları, kullanılan greft ve membran not edildi. İmplantların sağkalım oranları takip süreleri boyunca incelendi. Bulgular: İki grupta da 82 implant incelendi. İki grubun da sağkalım oranları aynı (%100) idi. Kemik yüksekliği değerlendirildiğinde iki grup arasında istatistiksel olarak anlamlı bir fark bulundu (p<0,001). Gruplar, takip süresi açısından istatistiksel olarak değerlendirildiğinde, iki grup arasında fark saptanmadı (p=0,415). Operasyonda görülen sinüs membran perforasyonu, implantların sağkalım oranını etkilememektedir (p=0,390). Sonuç: Lateral pencere tekniği ile eş zamanlı olarak implantlar yerleştirildiği zaman, 5 mm'lik bir kemik yüksekliği zorunlu değildir. İmplantın primer stabilizasyonu çok daha önemlidir. The aim of this study was to evaluate the survival rates of implants placed simultaneously with sinus augmentation at different bone heights under sinus floor. The primary determinant is the bone height below the sinus base, and the primary result is to evaluate the survival rates of the implants in this study. Material and Methods: Sixty-four patients (28 male, 36 female) who received implants simultaneously with sinus augmentation were included in this retrospective study. Patients were divided two groups according to the bone height of the sinus floor; Group 1 (<5 mm) and Group 2 (≥5 mm). Cone-beam computed tomography was used to measure bone height. The brands of implants, graft and membrane used were noted. Survival rates of implants were evaluated during follow-up. Results: 82 implants were evaluated in two groups. Survival rate was same in two groups (%100). There was a statistically significant difference between the two groups when the bone height was evaluated (p<0.001).
