Dermal Healing After Local Skin Flaps and Chemical Peel

Abstract: Like cells respond to injury through repair mechanisms unique for each cell type (eg, regeneration of the epidermis, granulation over exposed fat). How simultaneous chemical peel affects a local skin flap and why minimal differences in chemical peels can result in significant differences in outcome is not fully understood. In this study guinea pig skin was exposed to skin flap elevation, chemical peel, or a combination of the two. Biopsy specimens were taken at 3 to 35 days after injury and the histologic sect… Show more

“…Resurfacing performed at the same time or soon after face lifting or blepharoplasty increases the risk of skin necrosis and scarring, because of the altered blood circulation of the undermined skin following these procedures. 83 Thus, laser resurfacing of undermined skin should be performed at least 6 months after the original surgical procedure. Certain anatomic locations are associated with an unacceptably high risk of scarring, such as the hands, neck, and chest.…”

“…Of various available therapeutic modalities, none can be said to be ideal. 10 Therefore, to maximize the therapeutic effects of TCA and overcome the complications, such as scarring, hyperpigmentation, and hypopigmentation, a technique consisting of focal application of higher concentration TCA by pressing hard on the entire depressed area of atrophic acne scars using a sharpened wooden applicator, popularly known as CROSS, has been suggested by Lee and colleagues. 9 The beneficial effects of chemical peel are due to a combination of reorganization in dermal structural elements and an increase in dermal volume.…”
