Due to the prevalence of acne vulgaris, isotretinoin is one of the most prescribed drugs among physicians and dermatologists. Although exhibiting an adequate safety profile, adverse events secondary to isotretinoin use are common. Before prescribing isotretinoin, physicians usually inquire about pregnancy and perform serologic tests including cholesterol, triglycerides, and liver enzymes. Ocular manifestations are commonly neglected. Despite being generally mild, ocular manifestations related to either topical or systemic isotretinoin may cause important ocular morbidity. The ocular surface is the most affected site within the eye; however, retinal and optic nerve disease also have been documented. Evaporative dry eye disease, which may range from mild to severe, is the most common adverse ocular effect associated with isotretinoin use. The aim of this review is to present an up-to-date overview for the dermatologist about the prevention, diagnosis, and treatment of the ocular side effects of isotretinoin, and when to refer to the eye specialist.

Orofacial clefts (OFCs) have multiple etiologies and likely result from an interplay between genetic and environmental factors. Within the last decade, studies have implicated specific epigenetic modifications and noncoding RNAs as additional facets of OFC etiology. Altered gene expression through DNA methylation and histone modification offer novel insights into how specific genes contribute to distinct OFC subtypes. Epigenetics research has also provided further evidence that cleft lip only (CLO) is a cleft subtype with distinct etiology. Polymorphisms or misexpression of genes encoding microRNAs, as well as their targets, contribute to OFC risk. The ability to experimentally manipulate epigenetic changes and noncoding RNAs in animal models, such as zebrafish, Xenopus, mice, and rats, has offered novel insights into the mechanisms of various OFC subtypes. Although much remains to be understood, recent advancements in our understanding of OFC etiology may advise future strategies of research and preventive care.

Овчинникова РАН», Москва, Россия РЕЗЮМЕ В обзоре представлены данные зарубежных клинических исследований по изучению эффективности и безопасности применения системного изотретиноина (СИ) для лечения ряда дерматологических заболеваний. Рассмотрены механизмы действия ретинола и его производных, эффекты синтетических аналогов ретинола при заболеваниях, связанных с процессами кератинизации. В многоцентровых рандомизированных исследованиях показана эффективность СИ при лечении очаговой алопеции, генитальных папиллом, плоских бородавок, болезни Кирле и ряда других дерматозов. Определено место изотретиноина в системной терапии акне и розацеа. Даны рекомендации по ведению больных акне и розацеа в зависимости от формы и тяжести патологического процесса с применением СИ на основании современных отечественных и зарубежных рекомендаций. Широкий спектр активности (комедонолитический, противовоспалительный, себостатический, антипролиферативный, антиоксидантный) позволяет считать СИ препаратом первой линии в лечении тяжелых и резистентных форм акне и розацеа. Последние клинические исследования по СИ посвящены изучению влияния препарата на различные показатели безопасности терапии. Представлены данные о препаратах СИ нового поколения с повышенной биодоступностью, что позволяет снизить курсовую дозу и риск развития побочных эффектов. Показаны преимущества изотретиноина по технологии LIDOSE (комплаентность, эффективность, безопасность) при лечении акне и розацеа.
