Detection of circulating tumor cells for prediction of recurrence after adjuvant chemoradiation in locally advanced squamous cell carcinoma of the head and neck

Abstract: Assessment of CTC in non-OPC should prove useful for identification of patients who benefit from treatment intensification. The basis for the good prognostic value of CTC in OPC has to be elucidated in future studies.

“…However, validation in prospective cohorts is not yet available. So far, the prediction of therapy response by "liquid biopsy," analyzing CTC, tumor-derived circulating DNA, exosomes, or tumor-associated immune cells, could not be established in clinical routine [30,34,53,57,64] .…”

“…Interestingly, in the subset of OPCs within this dataset, CTCs were not predictive of disease free survival (DFS) or overall survival (OS). However, in non-OPCs, CTC were an independent prognostic marker of poorer DFS and OS [57]. Therefore, upon further validation, CTC quantification could prove to be a useful tool to identify patients who could benefit from treatment intensification.…”

“…However, in none of the three cases with persistent positive cytology was a visible tumor mass present. In patients with locally advanced squamous head and neck cancer, circulating tumor cells can be recovered from peripheral blood in which their number is related to stage of disease and may be associated with a worse prognosis following radical treatment . Although circulating tumor cells may serve as a marker for subsequent metastasis, their presence does not indicate that metastasis will definitely occur in those patients.…”
