Intersphincteric resection (ISR) has rapidly increased worldwide including laparoscopic surgery. However, there are some concerns for the definition of ISR, surgical technique, oncological outcome, anal function, and quality of life (QoL). The aim of the present study is to evaluate those issues. A review of this surgical technique was carried out by searching English language literature of the PubMed online database and appropriate articles were identified. With regard to open‐ISR, the morbidity rate ranged from 7.5% to 38.3%, with lower mortality rates. Local recurrence rates varied widely from 0% to 22.7%, with a mean follow‐up duration of 40–94 months. Disease‐free and overall 5‐year survival rates were 68–86% and 76–97%, respectively. Those outcomes were equivalent to laparoscopic‐ISR. Surgical and oncological outcomes of ISR were generally acceptable. However, accurate evaluation of anal function and QoL was difficult because of a lack of standard assessment of various patient‐related factors. The surgical and oncological outcomes after ISR seem to be acceptable. The ISR technique seems to be valid as an alternative to abdominoperineal resection in selected patients with a very low rectal cancer. However, both necessity for ISR and expectations of QoL impairment as a result of functional disorder should be fully discussed with patients before surgery.

Background and Aim: fecal incontinence quality-of-life scale (fiqls) is a conditionspecific health-related quality-of-life questionnaire composed of four scales: lifestyle, coping/behaviour, depression/self-perception and embarrassment. it has been widely translated and used as an evaluation tool for patients with fecal incontinence. our aim was to translate the fiqls, and to test some of the psychometric properties of the norwegian version of the questionnaire.Material and Methods: The fiqls was translated to norwegian, and administered to a sample of 76 patients (73 women) who completed the questionnaire at baseline and again after three weeks. in addition, the severity of incontinence was assessed by phoneinterviews (st. Mark's score).Results: Three of four domains had good internal consistency in terms of cronbach's alpha (.83-.91), the fourth (embarrassment) somewhat lower (.64). stability over time was acceptable for all domains with icc ranging from .74 to .86. correlation with severity of incontinence (st. Mark's score) was medium to large for all four domains (-.46 to -.63) supporting the construct validity of the norwegian fiqls.Conclusion: The norwegian version of fecal incontinence quality-of-life scale has been successfully translated and tested.

Transvaginal resection of a rectal leiomyoma: A case report

Abstract. The present study reports the case of a patient with a rectal submucosal tumor (leiomyoma) that was resected transvaginally. A 51-year-old female presented with a rectal submucosal tumor on the anterior wall of the lower rectum, located within 3 cm of the anal verge. This location would normally require intersphincteric or abdominal perineal resection. However, in order to minimize the invasiveness of the treatment and reduce post-operative morbidity, transvaginal resection and laparoscopic diverting ileostomy were performed instead. With the patient under general anesthesia, the posterior vaginal mucosa was incised vertically. The tumor was then excised en bloc with the overlying rectovaginal septum and rectal submucosal tumor. A primary repair of the defect and a diverting stoma were performed. The procedure did not present any complications, and the patient was discharged on day 10 post-surgery. The diverting stoma was closed 3 months later, and the sphincter function of the patient following surgery was monitored by manometry. The results of the manometric tests indicated that the patient did not suffer from fecal incontinence. In addition, the patient did not experience anal dysfunction or discomfort following the surgical procedure.
