Development and Validation of the Communication Behavior Scale for Nurses Caring for People with Dementia

(http://creativecommons.org/licenses/by-nd/4.0)If the original work is properly cited and retained without any modification or reproduction, it can be used and re-distributed in any format and medium.Purpose: The purpose of this study was to develop and validate the Communication Behavior Scale for nurses caring for people with Dementia (CBS-D). Methods: Based on communication accommodation theory, the initial items were generated through a literature review and interviews with 20 experts. Content and face validity of the initial items were assessed. Data from 486 nurses caring for people with dementia were analyzed using item analysis, exploratory and confirmatory factor analysis, criterion-related validity, and internal consistency. Results: The final scale consisted of 18 items and four factors (discourse response management, interpersonal control, emotional expression, and interpretability) that explained 57.6% of the variance. Confirmatory factor analysis indicated that the theoretical model with 18 items satisfied all goodness-of-fit parameters. Criterion-related validity was shown by the Global Interpersonal Communication Competence Scale (r=.506, p<.001). Cronbach's alpha for the total scale was .88. Conclusion: The CBS-D can be used to measure the communication behavior of nurses caring for people with dementia. 략이다[6-8]. 간호사는 바람직한 의사소통행위를 통하여 치매대상 자를 이해하고 그들이 의사를 표현하도록 도울 수 있다. 이를 통하 여 간호사와 치매대상자의 상호작용은 향상되고 결과적으로 치매대 상자의 건강은 증진될 수 있다[8]. 하지만 간호사는 치매대상자와의 적절한 의사소통을 위한 기술과 지식이 부족하고, 과도한 업무로 인 해 치료적 의사소통의 중요성을 간과하고 있는 실정이다[9]. 또한 간 호사는 치매대상자가 무능하고 타인에게 의존적이며 그들의 건강은 빈약하다는 부정적 고정관념에 기반하여 그들을 존중하지 않는 등 부적절한 의사소통행위를 보이기도 하지만[8], 이러한 사실을 제대 로 인식하지 못하고 있는 것으로 나타났다[10]. 따라서 간호사는 자 신이 치매대상자와 어떻게 의사소통행위를 하고 있는지 자기 자신에 대하여 지속적인 비판적 분석과 자기 검열이 필요하다[5]. 이를 위해 치매대상자를 돌보는 간호사의 의사소통행위에 대한 자가보고식 평 가도구가 필요하다. 선행 연구에서 치매대상자 돌봄제공자의 의사소통을 측정한 도구 를 살펴보면 신뢰도나 타당도가 모두 제시된 경우는 몇 편에 불과하 였다[11,12]. 또한 치매대상자 돌봄제공자의 의사소통을 측정할 때 의사소통 지식[13,14], 정서적인 음성[12], 일반적인 말하기[15] 등 의사소통의 일부 측면을 측정하고 있었다. 평가 방법 면에서는 관찰 을 통한 측정으로 평가를 위해 녹음이나 녹화 후 다시 확인해야 하 는 경우가 다수였다[12,15,16]. 최근 대만에서 치매대상자 돌봄제공 자의 의사소통역량을 측정하는 자기보고식 도구를 개발하여 타당도 와 신뢰도를 제시하였다[11]. 그러나 개발과정에서 근거가 되는 이론 적 모델과 전문가 집단과의 면담없이 의사소통 전략에 대한 문헌고 찰만을 토대로 하였으며, 치매대상자 돌봄제공자의 의사소통역량에 대한 정의를 명확히 제시하지 않은 제한점이 있는 실정이다. 치매대상자를 돌보는 간호사는 대상자와 의사소통할 때, 그들의 의사소통 장애 정도와 요구를 잘 파악하여 그 상태에 맞추어 조절 하는 행위를 해야 한다. 의사소통조절이론(Communication Accommodation Theory [CAT])은 다른 사람과 의사소통할 때 상대방과 자신의 심리적, 사회적, 언어적 차이에 대하여 정확히 알고 그 수준 과 상태에 맞추는 것을 전제로 한다[6]. 그러므로 의사소통조절이론 은 치매대상자를 돌보는 간호사의 의사소통행위를 적절히 평가할 수 있는 개념적 기틀을 제공할 수 있다. 또한 이 이론에 근거하여 의사 소통행위는 근접, 이해증진, 대화관리, 관계조절, 정서표현으로 분류 하여 평가될 수 있다[17]. 즉 간호사가 치매대상자의 대화양식에 맞 추면서 대상자의 이해증진을 도모하고 상호간 대화를 관리하며 대 상자와 동등한 관계에서 적절한 공감과 안심을 제공하는 행위[18]를 수행하고 있는지 세분화하여 의사소통행위를 확인할 수 있다. 이에 본 연구에서는 의사소통조절이론을 기반으로 치매대상자를 돌보는 간호사의 의사소통행위 측정도구(Communication behavior scale for nurses caring for people with Dementia [CBS-D])를 개 발하고 타당도와 신뢰도를 평가함으로써 치매대상자를 돌보는 간호 사의 의사소통행위를 증진...
