Device migration after endoluminal abdominal aortic aneurysm repair: Analysis of 113 cases with a minimum follow-up period of 2 years

Abstract: DM occurred in a significant portion of our patients, yet aggressive follow-up examination and a high reintervention rate prevented aneurysm-related death. According to our data, dilatation of the infrarenal aortic neck is an important factor that contributes to the distal migration of stent grafts, and patients with large aneurysms are at high risk for DM.

“…Greater degrees of endograft curvature will result in significantly larger forces applied to the endograft. While clinical endograft migration is usually described as longitudinal or caudal migration, 3,24,25 image analysis often reveals significant lateral movement of the endograft, as depicted in Figure 8. The 5.5-cm aneurysm was treated with an endograft, and postimplantation computed tomography (CT) showed good position of the endograft with complete aneurysm exclusion and no endoleak.…”

“…In addition, the possibility of endoleak and/or migration in the future cannot be ruled out because the number of patients in this study was small and the follow-up period was short. 56,57 …”

“…Thus, it is not surprising that migration has been noted with all current endovascular devices and configurations, including those with modular design, unibody design, infrarenal fixation, suprarenal fixation, hook and barb fixation, longitudinal columnar support, and flexible designs. [4][5][6][7][8][9][10] Migration has also been noted with prosthetic aortic grafts placed at open surgery. The risk of migration increases with time and can result in the loss of device fixation proximally, dis-tally, or at modular junctions, resulting in aneurysm rupture.…”

“…Unfortunately, the reported incidence of endograft migration varies widely between studies, and this may be due to different definitions of endograft migration and the follow-up period may vary. 4 …”
