Device-Related Thrombus After Left Atrial Appendage Closure

Abstract: After left atrial appendage closure with Watchman, DRT (≈3.7%) is not frequent but, when present, is associated with a higher rate of stroke and systemic embolism.

“…DRT is the Achilles heel of LAAC therapy, necessitating temporary resumption of OAC. DRT has an incidence of 2% to 5% and confers a threefold increased risk of stroke and systemic embolism . Risk of DRT is mitigated by device endothelialization, however, this process is difficult to assess and proceeds with variable kinetics.…”

“…Despite all, the DRT after LAAc is not frequent (3.7‐7.2%) but is associated with a higher rate of stroke and systemic embolism . Among those known to have DRT, the adjusted risk for stroke or systemic embolism was more than tripled compared with the risk without DRT . DRT (HR: 4.39; 95% confidence interval [1.05, 18.43]; P = 0.04) found an independent predictors of ischemic strokes and transient ischemic attacks during follow‐up .…”

“…Two plausible reasons for increased ischemic events after LAAc are reduction in the intensity of postimplant antithrombotic therapy and thrombogenicity of the intraatrial foreign device . Furthermore, little is known about the physiological consequences of LAAc.…”

“…A cohort assembled from four prospective studies including more than 1,700 patients implanted with the WATCHMAN closure device identified a 3.7% risk of device‐related thrombosis , associated with three times higher adjusted risk for stroke or systemic embolism and even higher risk of hemorrhagic stroke compared to patients without thrombosis. In about half the cases, the strokes seemed to occur within a month of device‐related thrombosis a plausible association.…”
