Diagnosis of biliary atresia with radionuclide hepatobiliary imaging.

Abstract: Sixteen patients with biliary atresia and 11 patients with neonatal hepatitis were studied preoperatively with either Tc-99m-diethyl-IDA or TC-99m-diisopropyl-IDA. Two parameters were evaluated: hepatocyte clearance and time to appearance of radioactivity in the intestine. Two observers, using a visual grading system of 1 to 4, gave the 16 patients with biliary atresia a hepatocyte clearance grade of 1.7 +/- 0.6 (mean +/- SD); intestinal radioactivity was not seen through 24 hours. The hepatocyte clearance gra… Show more

“…Both sexes are affected equally. No single test has proven 100% reliable in diagnosing BA (6,7). 99mTc trimethyl bromo iminodiacetic acids is considered best for Hepato-biliary Scintigraphy because of its 98% hepatic extraction and urinary excretion of 1.5% (8).…”

“…Gall bladder is seen as early as ten minutes and Bowel activity is by 30-40 minutes. The presence of tracer within the small bowel excludes BA (6). Most reviews report sensitivity of hepato-biliary scintigraphy for the diagnosis of BA between 97-100% and the specificity from 33-91% (3,6).…”

“…The presence of tracer within the small bowel excludes BA [12]. Most reviews report sensitivity of hepato-biliary scintigraphy for the diagnosis of BA between 97-100% and the specificity from 33-91% [3,6]. The present study being retrospective is not structured to yield comparable data.…”

“…All scans were reviewed and considered positive for Biliary Atresia ( Fig. 1) if the scans showed good liver uptake with no intestinal activity till 24 hr [6] or reduced background activity. These patients were then subjected to laparotomy and operative cholangiography and the findings correlated with scintigraphy.…”

“…Tc-99m IDA scan can rule out biliary atresia in an infant if patent biliary tree is shown with passage of radiotracer into bowel. The literature reports 91% accuracy, 97% sensitivity, and 82% specificity for hepatobiliary imaging in diagnosis of biliary atresia 4 .…”
