Diagnostic Value of Three Methods for Assessing Free Thyroxine in Pregnancy

Abstract: The clinical value of free thyroxine index, thyroxine/thyroxine binding globulin ratio, and free thyroxine concentration were compared in 46 pregnant patients. Overall the free thyroxine index was the best diagnostic index. Pregnancy induced changes in levels of free thyroxine and the thyroxine/thyroxine binding globulin ratio, which gave rise to interpretative problems in some pregnant subjects.

“…These original findings also demonstrate the dependence of serum FT4 on clinical state and drug treatment in geriatric patients. The influence of the serum TBG level is less evident. The serum FT4 level is, in principle, independ ent of TBG; yet in non‐thyroidal illness, both positive and negative correlations have been found 38,39 . In chronic geriatric patients, a weak inverse correlation of the “two step”‐FT4 values to TBG was observed.…”

“…A low TSH value in our case also indicated a status of hyperthyroidism before the caesarean section. Some reports indicate that the free T4 index is a reliable tool for the assessment of thyroid function during pregnancy [ 14 , 15 ]. Further clinical trials are needed to identify a practical marker to determine the level of thyroid hormones during pregnancy.…”

“…In addition, the FT4 index (FT4I) is a reliable marker of free thyroxine status during pregnancy, correcting TT4 for alterations in TBG [ 21 , 22 ].…”
