Differences between Cancer Patients’ Symptoms Reported by Themselves and in Medical Records

Abstract: Introduction: Data regarding rates of medical records concerning patients' symptoms are controversial. We aimed to calculate medical discovery rate of patients' symptoms and its association with symptoms severity.
