“…It has been reported that the health benefits gained from reducing complications and socioeconomic burden much outweighs the minimal risk of antibiotic treatment. Moreover, even the concern for development of antibiotic resistance is not justified since despite the large-scale use of penicillin, to date no resistant strains has yet developed [21], and even the minimum inhibitory concentrations have not changed in >70 years [60]. No doubt, that recurrent pharyngitis is a problem that requires an appropriate prophylactic antibiotic treatment, not only for its possible sequela if untreated but also for its marked global socioeconomic and health care burden.…”

“…This is simply due to its cost effectiveness, narrow spectrum of activity and long standing proven efficacy in elimination of GABHS and prevention of rheumatic fever [20]. Furthermore, according to a recent systematic review, to date, no penicillin-resistance has been documented [21].…”

“…Narrow-spectrum penicillins are the fi rst choice for treatment because of the rarity of documented resistance to penicillin by group A streptococcus during pharyngitis treatment and because of their low cost 2,6,23 . A recent literature review failed to show differences between antibiotics in treating streptococcal pharyngitis 23 .…”

“…Several attempts have recently been made to prove efficacy of shorter antibacterial therapy schemes; however, these recommendations have not gained wide acceptance [21]. Despite a steadily high GABHS cephalosporin-sensitivity, it is not advisable to use them as drugs of the first choice in view of several reasons: firstly, due to their higher cost in comparison with penicillins; secondly, due to the increased risk of antibiotic resistance formation in bacteria if cephalosporins are used [22]. Generation I-II cephalosporins may be used as an alternative therapy in patients with non-severe drug allergy to penicillins; however, they must not be used in presence of anaphylactic reactions to penicillins due to high risk of cross-sensitization [21].…”
