Different Therapeutic Approaches and Outcome in the Treatment of Pterygium

Abstract: Beta irradiation as a complement to surgical treatment of pterygium, is successful in treating high risk cases such as reoperations, whereas for the majority of primary pterygia surgical excision alone is adequate. Additionally, follow up of one year will reveal any recurrences.

“…[6][7][8][9][10][11] Conjunctival autografts are associated with relatively low recurrence rates (2-39%) and good cosmetic outcomes on long-term follow-up, and the application of mitomycin C with a conjunctival autograft has been shown to further reduce the recurrence rate of pterygia. 1,[12][13][14][15][16][17] However, mitomycin C is associated with scleral thinning or ulceration as a visionthreatening complication. 18,19 Pirfenidone (5-methyl-1-phenyl-2-[1H]-pyridone) is a new drug that has been reported to have antifibrotic effects in some experimental disease models, including pulmonary fibrosis [20][21][22] and liver cirrhosis.…”

“…9,[13][14][15][16][17] Even with the inclusion of limbal stem cells, recurrence rates of 0% to -18% were reported. However, later studies showed varying recurrence rates with free conjunctival autografting ranging from 2% to 39%.…”

“…However, the incidence of complications associated with the use of beta-irradiation therapy and of antifibroblastic substances (mitomycin C and oncotiotepa), especially late scleral necrosis, has been relatively frequent. [16][17][18] CONCLUSION There were no major complications during the study presented when 5-FLU was being used. This leads us to suggest that a single application of preoperative 5-fluorouracil 25 mg/ml for 5 minutes is effective and safe in the treatment of primary pterygium.…”
