Diffuse dermal angiomatosis: a variant of reactive cutaneous angioendotheliomatosis

Abstract: Reactive angioendotheliomatosis (RA) is a rare, benign disease. Affected patients present with self-limited, erythematous to violaceous plaques. The clinical lesions are due to intravascular hyperplasia of cytologically banal endothelial cells in the dermis. We report 2 patients who presented with ulcerated, violaceous plaques on the lower extremities. Both had severe peripheral vascular atherosclerotic disease requiring bypass grafts. Unlike previously described cases of RA, our patient's lesions were due to … Show more

“…Although DDA was described initially in 1994 as a variant of reactive cutaneous angioendotheliomatosis, 1 it was recently recognized as a distinct clinical pathologic entity in the spectrum of cutaneous reactive angiomatoses. 8 All the reported cases of DDA have involved women (age range, 28-63 years), with most of them in their 50s (Table).…”

“…The most efficient method for accomplishing this improvement is revascularization of the affected area. 1,6 Corticosteroid therapy was used in 1 patient, and the lesion completely resolved after 2 months. 2 Our patient had noted improvement with isotretinoin therapy, which was reported to be effective in 1 patient, possibly because of its antiangiogenesis effect.…”

“…RAE, on the other hand, has remained pathogenetically less well elucidated, although hypotheses have been proposed regarding RAE as a hypersensitivity reaction to foreign antigens suggested by its frequent association with infections [1, 2, 5, 12, 13, 14]. Moreover, an association with milk intolerance [13], luminal cryoprotein deposition [15], cold agglutinins [16], antiphospholipid syndrome [17]and severe atherosclerotic disease [18]has been described.…”
