Diffusion-Weighted Magnetic Resonance Imaging in Superior Sagittal Sinus Thrombosis

Abstract: Acute cerebral venous strokes may contain cytotoxic edema and/or vasogenic edema on DWI scans. DWI may be helpful in diagnosing cerebral venous thrombosis in cases with cryptic presentations.

“…We believe that stroke-like presentations with only slightly reduced or elevated ADC values on DWI scans should be treated with caution and raise the possibility of atypical causes such as venous stroke or nonvascular diseases ( Fig 5). 55 DWI of intravascular clots has been recently reported in the literature. 56 A hyperintense signal involving sinuses or veins on DWI (HSVdwi) was detected in 41% of the cases with CVT.…”

“…Diffusion-weighted MRI (DWI) can detect changes in random water diffusion (Brownian motion), offering a unique method to assess tissue damage in neurologic diseases with a higher sensitivity than conventional MRI. 8,14,[17][18][19][20][21][22] Pathologic processes that alter the normal barriers to water diffusion in the brain (axons, cell membranes, etc) result in increased or decreased water motion that can be detected by DWI in a quantitative fashion by determining the apparent diffusion coefficient (ADC). MS pathologic changes typically result in increased ADC values compared to normal individuals; such increases correlate with disease severity.…”
