Diffusion-weighted MRI of exercise-induced acute renal failure (ALPE)

Abstract: Acute renal failure with severe loin pain induced by anaerobic exercise (ALPE) is a rare condition that is accompanied by wedge-shaped contrast enhancement on computed tomography (CT) without evidence of rhabdomyolysis. In two pediatric cases with ALPE, we tried to determine the relationship between findings from CT and magnetic resonance imaging (MRI). Case 1 involved a 13-year-old Japanese girl with a diagnosis of ALPE with normo-uricemia. Contrast-enhanced CT after 24 and 48 h showed a wedge-shaped excretio… Show more

“…Previous studies of ALPE have noted the high sensitivity of T1-weighted imaging; for example, one case series noted localized signal hyperintensity in four of six patients ( 1 ), and another found signal hyperintensity in the kidneys of all three cases evaluated ( 3 ). A report of two pediatric ALPE patients showed clear wedge-shaped hyperintensity on DWI and slight signal hyperintensity on T2-weighted MRI ( 25 ), as seen in our case.…”

“…There are reports of similar findings [10, 28, 29], a report suggesting renal angina [10], and another indicating reversible renal vasoconstriction [30]. They support the active oxygen scavenging insufficiency/renal ischemia hypothesis.…”

“…Plain [29] or Doppler sonography [30] and Tc-99m DTPA renography [31] can have a diagnostic role in PRV. Recently, the role of diffusion-weighted MRI in the diagnosis of ALPE has been reported, and it showed wedge-shaped signal hyperintensity [32] . Contrast-enhanced ultrasonography with Levovist was reported as a diagnostic tool and revealed wedge-shaped filling defects [33] , [34] .…”
