A spiculated mass on a mammogram is highly suggestive of malignancy. We report the case of a 32-year-old woman with a radial sclerosing lesion that mimicked breast cancer on mammography. She visited her physician after palpating a lump in her left breast. Mammography showed architectural distortion in the upper inner quadrant of the left breast. Ultrasonography showed a low echoic area with an ambiguous boundary. Core needle biopsy was performed because of the suspicion of malignancy. Histological examination did not reveal any malignant cells. After 6 months, the breast lump became larger and the patient was referred to our hospital. Mammography performed in our hospital showed a spiculated mass, and therefore mammotome biopsy was performed. Histological examination revealed dense fibroelastic stroma with a wide variety of mastopathic changes, leading to a diagnosis of a radial sclerosing lesion. One year after the biopsy, the lump on her left breast had disappeared and mammography showed no spiculated mass.

Abstract. Benign breast diseases represent the vast majority of diagnosis in breast pathology. However, the limited capability in identifying lesions at high risk of breast cancer evolution is an increasing problem in clinical practice. In the present study, we tested the hypothesis that the overexpression of S100P calcium-binding protein, previously identified in the very early stages of breast carcinogenesis, could be used as a marker to differentiate lesions at high risk of malignant evolution. In addition to S100P, the well-known proliferative marker, Ki-67, and estrogen receptor (ER) status were also assessed by immunohistochemistry in 155 samples from patients who submitted to stereotactic vacuum-assisted core biopsy due to breast microcalcifications. Results showed a positive association between ER and S100P overexpression, as well as a clear positive association between S100P overexpression and high-risk lesions. The strong association between S100P and ER expression highlights the hypothesis about the possible role played by S100P in the very early stages of breast carcinogenesis.

Mammotome: Less Invasive than ABBI with Similar Accuracy for Early Breast Cancer Detection

Abstract. We performed a prospective analysis of two consecutive biopsy cohorts investigated by the same team to compare the Mammotome system with the ABBI procedure. From April 1997 to August 2003 a series of 413 nonpalpable mammographic lesions in 387 women (median age 56 years, range 30-84 years) were stereotactically biopsied in the University Hospital of Basel, Switzerland. Until October 1999 the ABBI system was applied exclusively, it was subsequently superseded by the Mammotome device in our clinic. Main outcome measures were accuracy, technical demand, and morbidity. Sensitivity (97.3%/96.8%), negative predictive value (99.2%/98.7%), and diagnostic accuracy (99.4%/99.1%) regarding the detection of malignancy were excellent for both techniques (ABBI/ Mammotome). The Mammotome procedure was faster and less invasive, thus causing significantly less morbidity. The larger specimen obtained by the ABBI procedure resulted in more detailed histology. In conclusion, recommend the Mammotome system as the method of choice for detecting nonpalpable early breast cancer.

Abstract: BP is a valid method for the diagnosis of nonpalpable breast lesions. In our experience, VACB is the method of choice because it is easy to perform and has high adaptability.
