Direct visualization of left ventricular free wall rupture by levocardiography

Abstract: Two cases of left ventricular free wall rupture and one case of combined left ventricular free wall and ventricular septal rupture are described where ventriculography played a key role in diagnosis. In all three cases of patients with acute myocardial infarction, identification and localization of the defect was made by angiography. This report illustrates the safety and feasibility of ventriculography in patients with suspected cardiac rupture.

“…These four cases, therefore, may be interpreted as an IMR that progressed to a FWR which transiently bled into the pericardium and became self-sealed or, conversely, as an IMR with a PE unrelated to FWR. Previous investigators have reported similar cases of PE with a myocardial channel filled with contrast material in the left ventriculogram 14,31 or in an echocardiogram, 6 in whom surgery confirmed the existence of a FWR. 6,14,31 Moreover, evidence of the progression of an IMR to a FWR in some cases has also been produced.…”

“…Otherwise, the sensitivity of echocardiography for diagnosing complications after AMI showed marked variation [9]. Some reports indicated that left ventriculography (LVG) was useful for identifying mechanical complications after AMI [10]. Performing LVG along with coronary angiography can provide immediate information of complications in the catheter laboratory and is important for preoperative patient assessment to consider additional surgical procedures.…”

“…In the adult population, free wall ruptures, ventricular septal defects, and pseudoaneurysms are acute or subacute processes. Left ventriculography can often provide the first suggestion of myocardial disruption which would guide further imaging and urgent surgical or catheter based treatment .…”
