Background: Dental caries and periodontal disease are bacterial infectious disease, mainly caused by plaque, a bacterial colony deposited on the tooth surface and gum tissue. Dental plaque disclosants easily stain the dental plaque, making them effective for scaling and tooth brushing education. As the erythrosine typically contained in dental plaque disclosants is highly cytotoxic, a low toxicity additive is needed. In this study, we aimed to examine the natural pigments with negligible cytotoxicity but can effectively stain the dental plaques for use in dental plaque disclosants. Methods: The pigmentation of eight types of natural pigments was tested on bovine tongue and teeth, as well as on head and neck tissue sections of experimental ICR mice. The cytotoxicity of gingival epithelial cells was measured via MTT assay. Pigmentation was performed on the bovine tongue and tooth surface. Pigmentation in the oral environment was observed in four mandibular incisors. A 2 Tone was used as a control. Results: Of the eight types of natural pigments, purple and blue pigments were effective in coloring dental plaques on the enamel surface as well as in the head and neck tissue sections. Additionally, purple and blue pigments were visible on the surface of the bovine tongue. Red, pink, orange, green, purple, and yellow pigments showed strong cytotoxicity, whereas brown and blue pigments had relatively low cytotoxicity. Blue pigment was effective in staining the dental plaque of four mandibular incisors. Conclusion: We suggest that the blue pigment derived from Gardenia jasminoides Ellis (Rubiaceae), which is effective for coloring dental plaques and has low cytotoxicity, is useful as a naturally derived dental disclosant.

Handheld optical coherence tomography for clinical assessment of dental plaque and gingiva

Significance: Optical coherence tomography (OCT) offers high spatial resolution and contrast for imaging intraoral structures, yet few studies have investigated its clinical feasibility for dental plaque and gingiva imaging in vivo. Furthermore, the accessibility is often limited to anterior teeth due to bulky imaging systems and probes. Aim: A custom-designed, handheld probe-based, spectral-domain OCT system with an interchangeable attachment was developed to assess dental plaque and gingival health in a clinical setting. Approach: Healthy volunteers and subjects with gingivitis and sufficient plaque were recruited. The handheld OCT system was operated by trained dental hygienists to acquire images of dental plaque and gingiva at various locations and after one-week use of oral hygiene products. Results: The handheld OCT can access premolars, first molars, and lingual sides of teeth to visualize the plaque distribution. OCT intensity-based texture analysis revealed lower intensity from selected sites in subjects with gingivitis. The distribution of the dental plaque after one-week use of the oral hygiene products was compared, showing the capability of OCT as a longitudinal tracking tool. Conclusions: OCT has a strong potential to display and assess dental plaque and gingiva in a clinical setting. Meanwhile, technological challenges remain to perform systematic longitudinal tracking and comparative analyses.

Peri-implant diseases, caused by bacteria from biofilm related to dental implants, are one of the main causes of late loss of implants. In this sense, peri-implant diseases are divided into periimplant mucositis, when it affects only the soft tissues, and peri-implantitis, when there is a bone involvement, which can lead to the failure of dental implant therapy. Thus, biofilm removal is essential for peri-implant health, allowing long-term success in implant therapy. To improve the visualization of oral biofilm, which is usually transparent or colorless, disclosing agents have been routinely used. However, disclosing agents have allergenic potential and can cause staining extrinsically in restorative and prosthetic materials, leading to aesthetic impairment. Thus, the use of fluorescence has been studied as an alternative for visualization of oral biofilm. Therefore, this report describes the use of wide-field optical fluorescence for visualization of oral biofilm associated with implants and teeth, in a routine appointment and follow-up of a partially edentulous patient with peri-implant mucositis. In addition, this report showed wide-field optical fluorescence can be used in a clinical routine of care of patients with dental implants. In this sense, wide-field optical fluorescence allowed easy and immediate visualization of the mature oral biofilm for its adequate removal, evaluation of the quality of restoration to sealing of screw access-hole of implant and identification of cariogenic lesions, without risk of allergic reactions or staining of prostheses and restorations.
