DISCOHAT: An Acronym to Describe the Spectrum of Symptoms Related to Bilateral Vestibulopathy

Abstract: Objective: To assess the prevalence of each symptom listed in the acronym DISCOHAT (worsening of symptoms in Darkness and/or uneven ground, Imbalance, Supermarket effect, Cognitive complaints, Oscillopsia, Head movements worsen symptoms, Autonomic complaints, and Tiredness) in patients with bilateral vestibulopathy (BVP), compared to patients with unilateral vestibulopathy (UVP).Methods: A descriptive case-control study was performed on BVP and UVP patients who were evaluated for their vestibular symptoms by t… Show more

“…Wenn Sie nach dieser Zeit bemerken, dass der Patient nicht zum Ende kommt und noch wichtige Informationen fehlen, können Sie an diesem Punkt mit gezieltem Nachfragen beginnen. Hier können Gedankenstützen wie SO STONED [ 17 ] und DISCOHAT [ 18 ] für eine systematische Erfassung der Informationen nützlich sein (Tab. 1 und 2 ; siehe auch „Herausforderung 3“ und „Herausforderung 4“).…”

“…Patients typically suffer from imbalance, worsening in the dark and/or on uneven ground, and movement-induced blurred vision (oscillopsia) ( 4 ). BVP also leads to additional symptoms such as an increased risk of falling, cognitive deficits, impairment of navigation and spatial memory, autonomic dysfunction, anxiety, and depression ( 4 11 ). Consequently, BVP leads to reduced quality of life and imposes a significant socioeconomic burden on society ( 12 14 ).…”

“…It is, therefore, imperative to improve the quality of history taking. To this end, various approaches have been proposed, e.g., asking key questions [15,16], focusing on time course and triggers (TiTrATE paradigm, [17]), on key dimensions (SO STONED paradigm, [18]), and on key symptoms (DISCOHAT paradigm, [19]).…”

“…In relation to the TiTrATE paradigm, the dimensions 'how often' and 'triggers' should be emphasized, since these are exactly the same as the 'time course and triggers' mentioned above; they too enable symptoms to be categorized into acute, episodic, or chronic vestibular syndromes. In addition, the DISCOHAT paradigm is specifically able to capture the wide spectrum of symptoms related to vestibulopathy (darkness worsens symptoms, imbalance, supermarket effect, cognitive complaints, oscillopsia, head movements worsen symptoms, autonomic complaints, tiredness) [19].…”
