Disparity analysis and prognostic value of pretreatment whole blood Epstein‐Barr virus DNA load and Epstein‐Barr encoding region status in lymphomas: A retrospective multicenter study in Huaihai Lymphoma Working Group

Abstract: Elevated Epstein-Barr virus (EBV) DNA load is common in lymphomas. However, it remains unclear whether the disparity in viral load and its prognostic value in lymphomas are correlated with Epstein-Barr encoding region (EBER) status. In this retrospective multicenter study, we collected the data of pretreatment whole blood EBV DNA (pre-EBV DNA) and EBER status and evaluated their disparity and prognostic values in lymphomas. A total of 454 lymphoma patients from December 2014 to August 2020 were retrospectively… Show more

“…Our results tie well with previous studies wherein the PEDL in different EBER groups [17]. Our data is similar to studies from Shen et al [17], nevertheless, the median pretreatment PEDL is much lower than that of them. It is worth noting that they have used whole blood samples including circulating tumor cells and other EBV-infected cells.…”

“…The prevalence of EBV is higher in developing countries, but our study embraces only a small number of studies in Africa and South America. In addition, although some studies have shown that EBV-DNA can be used as a prognostic marker for EBV-associated HL, the choice of compartments of peripheral blood and cut-off copies of EBV-DNA is different in various studies ( 37 , 51 , 77 , 78 ). Given the different criteria in the related original studies, we did not include the studies of using PCR method to detect EBV infection.…”

“…EBV infection is closely associated with BL, which leads to the di culties in treating refractory/relapsed BL patients. In addition, high EBV DNA load is associate with poor prognosis in many EBV positive lymphoma subtypes [1,4,5]. However, current treatment options ignored the EBV-positive status of BL due to di culty in eliminating the latent EBV [28].…”

“…Despite multi-drugs intensive chemotherapy has improved the prognosis of patients with BL to some extent, patients with refractory/relapsed BL still have poor prognosis, with long-term survival rate of only 10%-20% [2,3]. Moreover, lymphoma patients with high EBV DNA load were shown to have poor prognosis [4][5]. Unfortunately, the current chemotherapy regimens could not eliminate EBV.…”
