Disseminated Gonococcal Infection Presenting as Bacteremia and Liver Abscesses in a Healthy Adult

Abstract: Herein, we describe a bacteremia caused by Neisseria gonorrhoeae that presented as liver abscesses. The patient had no risk factors for disseminated gonococcal infection. Periodic fever, skin rashes, and papules were present and the results of an abdominal computed tomography scan indicated the presence of small liver abscesses. The results of blood culture and 16S rRNA sequencing of the bacterial isolates confirmed the presence of N. gonorrhoeae. The patient improved with antibiotic therapy.

“…In some instances, gonococcus can spread to the bloodstream, resulting in systemic manifestations which are called DGI – a common cause of clinic or hospital visits in sexually active young adults ( 3 ). Common manifestations of DGI are mild fever, rash, tenosynovitis, and polyarthralgia; however, uncommon manifestations such as myocarditis and liver abscess have been reported in literature ( 4 6 ). The rash typically has a maculopapular, pustular, necrotic, or vesicular appearance and occurs in the trunk, limbs, palms, and soles of feet ( 1 ).…”

“…Untreated gonorrhea can lead to disseminated gonococcal infection (DGI) when N . gonorrhoeae spreads to the blood or other parts of the body 9 . Gonococcal infections also can facilitate the transmission of HIV 10 .…”

“…Neisseria gonorrhoeae (N. gonorrhoeae ) is a Gram-negative diplococcus and an obligate human pathogen. Gonococci are the causative agents of the sexually transmitted disease gonorrhea and have the potential to enter the bloodstream and cause systemic disseminated infections with severe consequences, such as endocarditis and arthritis [ 28 , 67 , 68 ]. An increasing number of reports are highlighting the threatening development of multidrug-resistant gonococci; thus, new strategies are required to combat this pathogen.…”
