Do home pregnancy tests bring women to community health workers for antenatal care counselling? A randomized controlled trial in Madagascar

Abstract: Task-shifting the provision of pregnancy tests to community health workers (CHWs) in low-resource settings has the potential to reach significantly more underserved women at risk of pregnancy with essential reproductive health services. “…Participants in our cohort who received a pregnancy test were less likely to delay ANC-1. Pregnancy testing has been shown to increase early attendance at ANC in multiple low-resource contexts [ 41 , 44 , 45 ], which may be due to earlier pregnancy discovery [ 18 , 19 , 46 ]. Although our analysis did not specifically assess whether patients delayed care due to late discovery, our results suggest that access to pregnancy testing may increase early attendance.…”

“…Although our analysis did not specifically assess whether patients delayed care due to late discovery, our results suggest that access to pregnancy testing may increase early attendance. Home pregnancy testing has been successfully incorporated into CHW outreach efforts in other low-resource settings, and has been linked to earlier ANC attendance [ 45 , 47 ]. In Rwanda, where a robust CHW outreach program already exists but access to home pregnancy testing is limited, incorporating pregnancy testing in regular outreach efforts may be an effective strategy to increase early attendance at ANC.…”

Interventions endorsed by women to strengthen timely pregnancy awareness include the provision of free pregnancy test kits for home use, routine pregnancy testing by visiting community health workers and outreach education programs [ 8 10 ].…”
