Do Hospitals Rank Differently on HCAHPS for Different Patient Subgroups?

Abstract: Prior research documents differences in patient-reported experiences by patient characteristics. Using nine measures of patient experience from 1,203,229 patients discharged in 2006-2007 from 2,684 acute and critical access hospitals, the authors find that adjusted hospital scores measure distinctions in quality for the average patient with high reliability. The authors also find that hospital "ranks" (the relative scores of hospitals for patients of a given type) vary substantially by patient health status an… Show more

“…However, some variability was found for the family physician's respectful treatment and communication with the practice assistant. These findings are in line with U.S. studies (Elliott et al, 2010;Rodriguez et al, 2008). So despite the differences that exist between the two countries in terms of the population and the health care system, between-provider variability of case-mix associations mainly relate to conduct measures.…”

“…Significant variation between practices in case-mix associations was found for age, education, general health status, and ethnicity, which corresponds to findings from previous studies (Elliott et al, 2001;Elliott et al, 2010;Lurie et al, 2003;Rodriguez et al, 2008). The well-known response biases (older, healthy people born in the Netherlands and with lower levels of education reporting more positively) were not present for all practices, nor was the size of the associations consistent across practices.…”

“…More recently patient-reported measures, such as patient satisfaction have emerged and are increasingly used to monitor and guide quality improvement efforts [1] in the United States (US) [2] and Europe [3] with evidence of success [4] and improved outcome through better continuity of care and compliance. [5,6] The shift to increased patient involvement has witnessed a migration from paternalistic approaches where clinicians dominate and decide, [7] to more active patients with clinician partnership, exchanging information, detailling preferences and deciding on options together, [8] or shared decision-making.…”

“…Selfrated health status was associated with patient ratings of care, quality, and satisfaction, [15][16][17] but Elliott et al 18 suggested that these effects within individual hospitals are likely very small. These differences likely affect hospitals with a diverse population, which makes between-hospital comparison difficult.…”

“…These differences likely affect hospitals with a diverse population, which makes between-hospital comparison difficult. 18 In several studies, self-perceived health status was associated with patient ratings of health care satisfaction. In one study, actual admitting diagnoses were used to understand the association between health problems and perceptions of care.…”
