Does Anterior Cruciate Ligament Reconstruction Protect the Meniscus and Its Repair? A Systematic Review

Abstract: Background: Anterior cruciate ligament (ACL) tear and meniscal injury often co-occur. The protective effect of early ACL reconstruction (ACLR) on meniscal injury and its repair is not clear. Critical literature review can support or change clinical strategies and identify gaps in the available evidence. Purpose: To assess the protective effect of ACLR on the meniscus and provide clinical guidelines for managing ACL tears and subsequent meniscal injury. We aimed to answer the following questions: (1) Does ACLR … Show more

“…One possible explanation of inferior knee function in patients opting for initial nonreconstructive treatment but subsequent ACL reconstruction versus early ACL reconstruction might be related to subjecting the knee to further damage, especially in the setting of increased laxity. 2 , 11 It is important to acknowledge the risk of confounding by indication, as this study is a follow-up of ACL injuries registered in the SNKLR and does not provide information on how or why patients received a specific treatment for their ACL injury. There may also have been differences between treatments and patients that explain the results, such as the level of participation in sport and the amount and quality of rehabilitation.…”

“…Secondly, patients with LMPRTs are often concomitantly treated with an ACL reconstruction. This could affect the outcome because the prospects of meniscal healing in general is improved when ACL reconstructions are performed in the same setting [ 19 ]. At last, lateral menisci with posterior root tears are less prone to excessive extrusion and the contact areas in the joint less diminished after LMPRT compared to MMPRT due to the meniscofemoral ligaments [ 2 , 11 ].…”

“…Bezüglich der Protektion von sekundären Meniskusschäden zeigen 4 Übersichtsarbeiten bessere Ergebnisse für eine operative Stabilisierung des Gelenks [13,38,44,84]. Leider werden in diesen Arbeiten in vielen Fällen nicht operativ versorgte Patienten mit konservativ Therapierten verglichen, sondern früh rekonstruierte Patienten mit spät rekonstruierten Patienten.…”
