Does Continuous Venovenous Hemodiafiltration Concomitant with Radiological Procedures Provide a Significant and Safe Removal of the Iodinated Contrast Ioversol?

Abstract: Background: The purpose of our study was to investigate whether continuous venovenous hemodiafiltration (CVVHDF) concomitant with radiological procedures (RxP) was feasible, well tolerated and could significantly remove iodinated contrast media (CM). Methods: 26 patients with various degrees of renal insufficiency who were submitted to RxP were included in the study. The CVVHDF session was started immediately before CM administration. All the patients were evaluated for feasibility and tolerability; furthermor… Show more

“…Continuous renal replacement therapies (CRRT), hemodiafiltration (CVVHD), and hemofiltration (CVVH) have also been studied [122][123][124]. When CVVHD was initiated immediately before and continued throughout the contrast study, no effect on the incidence of CIN was observed [122].…”

“…When CVVHD was initiated immediately before and continued throughout the contrast study, no effect on the incidence of CIN was observed [122]. In the first of two CVVH study by Marenzi et al [123], prophylactic hemofiltration was begun 6 h before contrast administration in a high-risk group (serum creatinine 3.0 mg/dl), held up during the angiography procedure, and then resumed following the procedure and continued for 18-24 h [123].…”

“…Amongst those, three studies utilized periprocedural hemodialysis, with variable benefit [258][259][260][261]. Prophylactic CVVH was evaluated in a total of 114 patients at high risk of contrast nephropathy undergoing coronary intervention.…”

“…However, this review included two nonrandomized trials [20,21], and heterogeneity was noted across the trials. In the present study, we combined evidence from nine RCTs to obtain current estimate on the effect of RRT for prevention of CI-AKI.…”
