Does cooled dialysate still have a role in reducing intradialytic stress? Implications of the MyTEMP trial

Katherine L. Hull,
Chris McIntyre,
James O. Burton

Abstract: Purpose of review There is an excess of cardiovascular morbidity and mortality in the maintenance haemodialysis population. Targeting traditional risk factors (e.g. hypercholesterolaemia) do not improve cardiovascular outcomes. Repeated myocardial stunning during haemodialysis is an important nontraditional risk, resulting in pathological cardiac remodelling and fibrosis. This review explores dialysate cooling as a management strategy to promote haemodynamic stability, reduce myocardial injury, and… Show more

“…135 Further detailed discussion relating to the current status of dialysate cooling is available in a number of targeted detailed editorials and reviews. 137,138 The potential value of this intervention clearly requires further study to address both patient selection and optimum delivery. Questions to examine include whether benefit is apparent in incident patients (before established nonreversible cardiac injury has set in) or in vulnerable subpopulations ( i.e ., intradialytic hypotension, high ultrafiltration requirements).Improvement of tissue ischemic tolerability: This can be achieved by preconditioning.…”
