Aims: The study aimed to assess the diagnostic value of serial monitoring of biochemical inflammatory markers (C-reactive protein (CRP) and white blood cell (WBC) count) in the postoperative diagnosis of anastomotic leakage or ureteral injury after bowel resection for deep infiltrating endometriosis. Methods: This is a review of prospectively collected data from 217 patients who underwent laparoscopic bowel resection for endometriosis from January 2009 to April 2015. Patients with an anastomotic leakage or ureteral injury were identified and classified. Results: The frequency of anastomotic leakage requiring reoperation was 8.3%. The frequency of ureteral injury was 4.6%. Median time to diagnosis was 6 days for anastomotic leakage and 8 days for ureteral injury. The daily mean values of serum CRP were significantly higher in patients with a surgical complication starting at the second postoperative day (POD 2, p = 0.004). WBC was significantly higher (p < 0.05) on POD 2 and 3 in patients with a surgical complication. A decrease in CRP from POD 1 to 3 predicted an uncomplicated course in 92.1% of the cases. Conclusion: A decrease in CRP from POD 1 to 3 was an indicator of uncomplicated subsequent postoperative course. The test is recommended when early discharge after rectal resection for deep infiltrating endometriosis is considered.

Background: The identification of presurgical clinical markers may be helpful to allow the staging of endometriosis severity. It has been suggested that pain characteristics orientate the gynecologist about the anatomical involvement of endometriosis. The study was performed to analyze the correlation between pain symptoms and the anatomical location of endometriosis. Methods: One hundred fifty-five consecutive patients with a complete removal of deep infiltrating endometriosis (DIE) were included. Prior to surgery, data on patient and disease characteristics were obtained. The intensity of the pain symptoms was registered using a Visual Analogue Scale. The endometriotic lesions were categorized according to the Enzian morphological classification. Correlation and multivariate analysis were performed to assess the potential associations between pain characteristics (dysmenorrhea, pelvic pain, dyschezia, dyspareunia or dysuria) and the location of endometriosis or other disease-related characteristics (hematuria, rectal bleeding or adenomyosis). Results: Pelvic pain was significantly associated with the presence of adenomyosis. Dyschezia was correlated with rectal bleeding and dysuria with the presence of hematuria. No relationship was found between other kinds of pain and the morphological location of endometriosis or other disease-related characteristics. Conclusion: Our data suggest that pelvic pain is correlated with the presence of adenomyosis in women with DIE. Further studies are required.

Endometriozė -dažna reprodukcinio amžiaus moterų liga, sukelianti skausmą ir nevaisingumą. Chirurginis gydymas yra vienas iš svarbiausių endometriozės gydymo etapų. Tinkamas chirurginis gydymas palengvina endometriozės sukeliamus simptomus ir padidina pastojimo dažnį gydant nevaisingumą. Laparoskopinės chirurgijos technikų ir taktikų tobulėjimas lėmė, kad endometriozės chirurginis gydymas tapo svarbiu gydymo etapu, turinčiu pranašumų, palyginti su medikamentiniu gydymu. Skiriamos trys pagrindinės ligos formos: pilvaplėvės, kiaušidžių ir gilioji rektovaginalinės pertvaros endometriozė. Paprasčiausia atlikti paviršinių endometriozės židinių abliaciją, o gydant giliąsias ligos formas židinių ekscizijos būdu reikalinga chirurgo patirtis ir įgūdžiai. Endometriozės chirurginis gydymas yra tausojantis arba radikalus, jei šalinama gimda ir (ar) kiaušidės. Labai svarbu laparoskopijos būdu nustačius endometriozę pašalinti visus matomus pažeidimus. Pagrindinis šio straipsnio tikslas yra padėti suprasti chirurginio gydymo svarbą gydant endometriozę ir pateikti moksliškai pagrįstus įrody-mus, kaip tinkamai gydyti endometriozę chirurginiu būdu.
