Double Orifice Mitral Valve With Flail Leaflet: a Transoesophageal Echocardiographic Examination

Abstract: We describe a case of double orifice mitral valve and a flail leaflet in a 54-year-old man. This rare congenital abnormality was disclosed through the discovery of a murmur. The transoesophageal approach clearly showed two approximately equal orifices with multiple papillary muscles. Colour Doppler echocardiography showed a moderate mitral regurgitation due to the prolapse of the posterior leaflet of the anteromedial orifice. No other abnormality was associated. Transoesophageal echocardiography is useful to a… Show more

“…4 The prognosis for patients with a DOMV in terms of valvular function has not been clearly defined, but the medical literature reveals that although both orifices may be functional in less than half of adolescent patients, both are rarely functional in adults. [4][5][6] No consensus opinion has been expressed in the medical literature about the need for antibiotic prophylaxis in cases of DOMV. In keeping with what holds for patients with other cardiac structural lesions that are not hemodynamically abnormal, our patient would have been considered to be at low risk for infective endocarditis on the basis of her DOMV alone; however, antibiotic prophylaxis was prescribed for her because of the concomitant presence of the mildly stenotic bicuspid aortic valve.…”

“…DOMV is a rare congenital valvular malformation with a relatively low incidence (there is no clear statistics on the disease incidence available so far), and has been only occasionally reported 2‐6 . The mitral valve originates from the myocardium compaction, the failure of which is considered to be the origin of the noncompact myocardium (NVM).…”

“…Двойное устье МК. ДУМК описано впервые Greenfield WS в 1876г [17]. Аномалия формируется в результате появления дополнительного отверстия в передней створке МК или в случае образования широкой перемычки между передней и задней створкой с двумя отверстиями в каждой створке (рис.…”
