Drug Repurposing and Observational Studies: The Case of Antivirals for the Treatment of COVID-19

Abstract: Remdesivir and molnupiravir received emergency use authorization on the basis of single trials launched after observed in vitro activity against SARS-CoV-2. For tenofovir disoproxil fumarate (TDF), little in vitro evidence was generated, and no randomized trials for early treatment were done. Yet, observational evidence suggested a lower risk for severe COVID-19 in TDF users compared with nonusers. The authors review the decision-making process for these 3 drugs and propose better use of available observationa… Show more

“… 54 , 55 Several antiviral drugs were repurposed for the treatment of COVID-19 including remdesivir, molnupiravir and tenofovir. 56 Other drugs were part of clinical trials, such as hydroxychloroquine and lopinavir/ritonavir but were withheld from the guidelines due to lack of evidence for effectiveness. 57 Remdesivir was effective in reducing hospitalization rates and mortality among patients with COVID-19 and was reserved for patients with severe disease who were hospitalized.…”
