Background and AimThe clinical association between dry eye syndrome (DES) and chronic fatigue syndrome (CFS) remain unclear with less evidences. We aimed to investigate the relationship between CFS and DES using a national insurance and prospective cohort study.MethodsData from the Longitudinal Health Insurance Database 2000 was applied to estimate the incidence of CFS among patients with DES, and their age- and sex-matched controls without DES over a long-term follow-up period. All participants were CFS free at baseline, before the interval (2005–2007), but were later diagnosed with CFS. DES patients and its relative matched controls were excluded prevalent CFS before the same interval.ResultsWe identified 884 patients with DES and 3,536 matched controls in baseline and estimated the hazard ratios for incident CFS in the follow-up period. Patients with DES had a 2.08-fold considerably increasing risk of developing CFS, compared to non-DES group. An elevated risk of developing CFS remained (1.61-fold risk) even after adjusting for age, sex, and comorbidities. There was a presence of increasing risk in DES-related CFS when CFS-related comorbidities existing (adjusted hazard ratio, 1.98, 95% confidence interval, 1.19–3.29; p < 0.01). The subsequent risk for CFS between DES and non-DES patients was significant increased with three or more annual medical visits, the adjusted risk for CFS was 4.88-fold risk (95% CI, 2.26–10.58, p < 0.001).ConclusionWe recommended that physicians should be aware of the increased risk of CFS among DES patients and adequately assess the health impacts among these patients.

Objective: Fibromyalgia (FM) is a rheumatic disease which exactly unknown etiology, causes widespread body pain. In FM patients, a large variety of ocular findings, especially dry eye, occur with and without other rheumatic diseases. These ocular findings may be the first findings to warn the clinician for diagnosis in rheumatic diseases. The aim of this study is to evaluate the eye findings as clinically in patients with FM. Materials and Methods: The patients of those between the ages of 18-75, who were admitted to the Aksaray University Training and Research Hospital, Physical Medicine and Rehabilitation Polyclinic and diagnosed with FM, and then examined in the Ophthalmology Polyclinic, were included in the study. Patients' files were reviewed retrospectively. The clinical diagnosis, socio-demographic characteristics, ocular examination findings, history, family history and the results of the treatments were recorded. Those who had undergone eye surgery, rheumatologic disease, diabetic retinopathy, glaucoma, systemic infection, autoimmune disease, cerebrovascular disease, major depression, abnormal laboratory values, and history of malignancy, were not included in the study. The diagnosis of FM was determined according to the American College of Rheumatology updated in 2016. Results: The files of 94 patients who met the appropriate criteria were included in the study. The mean age of the patients was 38.2 (18-75), and 80 (85%) were female and 14 (15%) were male. According to the ocular findings, 57 patients (57.4%) had eye pain, 28.2% had redness, 26.9% had burning-stinging, 15.1% had turbidity, 6.3% had burring, and 3.7% had itching. Among the causes of eye pain were 49.1% dry eye, 24.6% refractive failure, 12.3% blepharitis, and 3.5% allergy, while no cause was found in 10.5%.

Dry eye disease (DED) has become common on a global scale in recent years. There is a wide prevalence of DED in different countries based on various ethnicities and environment. DED is a multifactorial ocular disorder. In addition to advanced age and gender, such factors as living at high altitude, smoking, pterygium, prolonged use of consumer electronics or overingesting of caffeine or multivitamins are considered to be the major risk factors of DED. We report the DED epidemiology in Taiwan firstly in this article. According to the pathophysiological factors and changes inthe composition of the tear film in DED, it can be categorized into several subtypes, including lipid anomaly dry eye, aqueous tear deficiency, allergic and toxic dry eye among others. Each subtype has its own cause and disease management; therefore, it is important for ophthalmologists to identify the type through literature review and investigation. The management of DED, relies not only on traditional medications such as artificial tears, gels and ointments, but also newer treatment options such as acupuncture, SYL1001, and nanomedicine therapy. We also conducted a comprehensive literature review including common subtypes and treatment of DED. Clearly, more clinical trials are needed to assess the efficacy and safety of the various treatments and common subtypes of DED.
