Abstract. Malignant pleural mesothelioma (MPM) has a poor prognosis, and conventional imaging modalities do not reflect the prognosis of MPM. In this study, the clinical significance of 18 F-fluorodeoxyglucose positron emission tomography/ computed tomography fusion imaging ( 18 F-FDG PET/CT) was evaluated for the differential diagnosis, staging and prognosis in MPM patients. Ninety patients who underwent 18 F-FDG PET/CT scanning due to a clinical diagnosis or suspicion of MPM prior to therapy were reviewed. Of 90 patients, 31 were pathologically diagnosed as MPM. Maximum standardized uptake values (SUVmax) were semi-quantitatively obtained from PET/CT 60 min (early phase) and 120 min (delayed phase) after injection of 18 F-FDG, and the clinicopathological correlations with the level of SUVmax obtained from PET/CT were examined. The survival curves of MPM patients were plotted according to the methods of Kaplan-Meier. The prognostic implications of the level of SUVmax were estimated by t-test. PET/CT scan showed intense abnormal FDG uptake (SUVmax >2.0) in the pleural lesions of all 31 MPM patients at delayed phase, while it showed abnormal FDG uptake in 30 (97%) patients at early phase. In all 31 MPM patients, the values of SUVmax at delayed phase were higher than those at the early phase. PET/CT also indicated metastasis in the lymph node in 7 patients (23%) and in the systemic lesions in 8 patients (26%) with MPM. Twenty-three MPM patients with high SUVmax, whose prognosis was apparent, showed significantly poorer prognosis in both early and delayed phase (respectively, p=0.03 and p=0.01, t-test). The results showed that 18 F-FDG PET/CT at delayed phase is very useful for the diagnosis of pleural diseases, and SUVmax on PET/CT in the delayed phase is a more reliable prognostic factor than that in the early phase. High uptake of 18 F-FDG PET/CT may be a predictive factor of prognosis in MPM patients. IntroductionThe most common cause of pleural effusion is nonspecific pneumonia which can be successfully treated with proper antibiotic therapy. However, pleural effusion may persist and require further examinations in some cases in spite of proper therapy. Computed tomography (CT) provides detailed information on morphological changes, but on its own cannot differ entiate between benign and malignant diseases. Also, invasive pro cedures are frequently required for a definitive diagnosis (1). Malignant mesothelioma (MM) is a fatal malignancy with a rapidly increasing incidence in industrialized countries because of the widespread use of asbestos in the past centuries. Early diagnosis of MM is critical for a better prognosis, but this is often difficult because of the lack of disease-specific diagnostic imaging. Here, we report that manganese-enhanced magnetic resonance imaging (MEMRI) represents a promising approach for a more selective mesothelioma imaging by monitoring a high-level expression of manganese-superoxide dismutase (Mn-SOD), which is observed in many MM. We found that most human MM cells overexpressed Mn-SOD protein compared with human mesothelial cells and that NCI-H226 human MM cells highly expressed Mn-SOD and augmented Mn accumulation when loaded with manganese chloride (MnCl 2 ). The cells showed marked T 1 -signal enhancement on in vitro MRI after incubation with MnCl 2 because of the T 1 shortening effect of Mn 21 . H226subcutaneous tumor was preferentially enhanced compared with a lung adenocarcinoma cell tumor and another human MM cell tumor in MnCl 2 -enhanced T 1 -weighted MR image (T 1 WI), correlating with their respective Mn-SOD expression levels. Moreover, in a more clinically relevant setting, H226 xenografted pleural tumor was markedly enhanced and readily detected by MEMRI using manganese dipyridoxyl diphosphate (MnDPDP), a clinically used contrast agent, as well as MnCl 2 . Therefore, we propose that MEMRI can be a potentially powerful method for noninvasive detection of MM, with high spatial resolution and marked signal enhancement, by targeting Mn-SOD.Malignant mesothelioma (MM) is an asbestos-related malignant tumor originating from mesothelial cells lining pleural and peritoneal cavities. There is a long latency period (more than 30 years) between the asbestos exposure and the onset of the disease, and the number of patients is now rapidly increasing in industrialized countries because of the widespread use and installation of asbestos fibers in the past centuries. Epidemiological studies indicate that the incidence is rapidly increasing worldwide.

In the last 10 years, Fluorine-18 fluorodeoxyglucose (FDG) Positron Emission Tomography (PET) scan has been widely adopted in thoracic oncology, showing high accuracy in staging and restaging. The aim of this review was to focus on the results of PET scan use in the evaluation of thoracic malignancies. The literature search was performed on January 2015 in PubMed and Google Academic. The search was restricted to publications in English, using in the same string the word "PET" with different thoracic tumors. From 3000 papers initially selected, the irrelavant articles eliminated and then approximately 100 manuscripts were fully analyzed. In conclusion, positron emmision tomography/ computed tomography (PET-CT) has established itself as an important step in the evaluation of patients with thoracic malignancies. Further evaluation and come into use of newer PET tracers and simultaneous Magnetic Resonance Imaging (MRI) with PET (PET/MR) imaging will offer additional options in thoracic oncology in future.
