Dynamic Optical Coherence Tomography Is a New Technique for Imaging Skin Around Lower Extremity Wounds

Abstract: Chronic wounds such as venous leg ulcers invariably heal slowly and recur. In the case of venous leg ulcers, poor healing of chronic wounds is variously attributed to ambulatory hypertension, impaired perfusion and diffusion, presence of chronic inflammation at wound sites, lipodermatosclerosis, and senescence. The aim of this study was to investigate whether a new technique, optical coherence tomography (OCT), which permits imaging of blood capillaries in the peri-wound skin, can provide new insights into the… Show more

“…OCT and angiographic OCT can also provide objective metrics for the assessment of chronic wounds including burns, 86,87 and ulcers resulting from the genetic condition epidermolysis bullosa, where there is a particular concern of SCC formation. Here, OCT has been shown to aid in wound assessment and in reducing requirements for skin biopsy 88 .…”

“…Venöse Ulzera wurden mit der D-OCT an den 4 Wundrändern (mit Überlappung in das Wundbett), im Wundbett, 2 cm entfernt von der Wunde und am nicht betroffenen kontralateralen Bein auf gleicher Höhe gemessen (▶ Abb. 4 b) [10]. Am Wundrand zeigen sich aufgrund des erhöhten Blutflusses große geknäuelte Blutgefäße ("coils") (▶ Abb.…”

“…In the current issue, there is, again, a wealth of progress in the science of wounds. Among others, Holmes et al 12 report favorable results with dynamic optical coherence tomography in the evaluation of chronic ulcers. Luck et al 13 summarize the evidence from 11 randomized controlled trials on the utility of allogeneic skin substitutes compared with human placental membrane products for diabetic foot ulcers.…”
