Dysfunctional Immune Regulation in Autoimmune Hepatitis: From Pathogenesis to Novel Therapies

Abstract: Autoimmune hepatitis (AIH) is a chronic inflammatory disorder characterized by hypergammaglobulinemia, presence of serum autoantibodies and histological features of interface hepatitis. AIH therapeutic management still relies on the administration of corticosteroids, azathioprine and other immunosuppressants like calcineurin inhibitors and mycophenolate mofetil. Withdrawal of immunosuppression often results in disease relapse, and, in some cases, therapy is ineffective or associated with serious side effects. … Show more

“…The imbalance between pro-inflammatory mechanisms and regulatory ones is presumed to play a pivotal role in the pathogenesis of autoimmune diseases in general and autoimmune hepatitis in particular 8788. The activities of the regulatory cells, Th1 cells, Th17/Th22 cells, activated macrophages, complement, and natural killer cells are all interconnected and finely tuned, and when such a system becomes dysfunctional, the autoimmune disorder may ensue 89.…”

“…The trigger underlying the switch towards an overwhelming immune response against autoantigens is still unknown. From an immunological view, the autoantigen presentation by conventional and unconventional liver APCs, the unsuccessful control of autoreactive T cells and the production of autoantibodies play a crucial role ( 43 , 44 ). Dysregulated Treg cells which are also susceptible to Fas ligand-mediated apoptosis can be observed within the liver of AIH patients ( 45 48 ).…”

“…Treg cells are key players in autoimmune diseases including AIH. Functional impairment and altered frequency of Treg cells in AIH patients cause breakdown of self-tolerance mechanisms ( 46 ) with consequent beginning and progression of autoimmune liver injury ( 44 , 65 ). In patients with AIH, the frequency of circulating Treg cells and their ability to suppress T cell proliferation are lower than in healthy subjects, especially at diagnosis and during relapses than during drug-induced remission ( 66 68 ).…”

“…Unfortunately, there are currently no data available to this study ( ). Clearly such studies are aiming at the re-establishment of a healthy immune balance by enhancing the regulatory side, which has been shown to be impaired in patients with AIH ( 42 , 43 ). In contrast, the immune balance can also be restored by dampening the aggressive side via a direct targeting of activated, autoaggressive T cells.…”
