Early decreased TLR2 expression on monocytes is associated with their reduced phagocytic activity and impaired maturation in a porcine polytrauma model

Abstract: In their post-traumatic course, trauma patients suffering from multiple injuries have a high risk for immune dysregulation, which may contribute to post-injury complications and late mortality. Monocytes as specific effector cells of the innate immunity play a crucial role in inflammation. Using their Pattern Recognition Receptors (PRRs), notably Toll-Like Receptors (TLR), the monocytes recognize pathogens and/or pathogen-associated molecular patterns (PAMPs) and organize their clearance. TLR2 is the major rec… Show more

“…Physiological trauma was shown to be associated with a decreased expression of TLR2 and HLA-DR on monocytes in the first 5.5 hours after induction in a post-traumatic porcine model. The reduced expression of both surface molecules was accompanied by immune dysregulation, which was demonstrated by reduced phagocytic activity in this model 41 .…”

“…Interestingly, large animal models are gaining more and more interest in the context of PT 6870 . Because porcine hemodynamic responses are comparable to those in humans, such trauma models have been established 71,72 . Although pigs are easier to handle during surgical procedures, far more technical equipment and increased financial support are required 73 .…”

“…Recent publications indicate that a relevant number of SARS-CoV-2-infected patients may develop severe inflammatory complications with diffuse pulmonary inflammation and alveolar damage [ 17 , 18 ]. Some reports even document the presence of a cytokine storm in the infected patients leading to altered cellular and humoral immune responses [ 19 – 21 ], similar to post-traumatic acute lung injuries (ALIs) induced by both direct and non-direct pulmonary insults [ 22 , 23 ]. Besides the assessment of the overall conditions of trauma patients (age, injury severity, pattern, physiological response), the presence of bilateral opacities in chest imaging may influence the decision-making in trauma bay, as well.…”
