Early donor management increases the retrieval rate of hearts for transplantation in marginal donors†

Abstract: In our study, we were able to retrieve more than half of the potential heart donors as a result of early active donor management without impacting on the post-transplant recipient outcome. Early active donor management may assist in increasing the number of heart transplantations, thus warranting further investigation.

“…105 Early, targeted management of the potential donor can increase the number of available donor hearts without a deleterious effect on post-transplant outcomes. 107 A number of studies demonstrate improved organ donation rates after a longer period of donor stabilization. 108−111 A dedicated team in an ICU setting should manage the pediatric cardiac donor to maximize the probability of successful transplantation.…”

“…Many donor lungs have become unsuitable for transplantation at the time of offer due to catecholamine and inflammatory responses causing detrimental damage to the donor lungs ( 14 ). During the period between confirmation of brain death and organ retrieval, attention to detail and optimization may improve organ function, thereby increasing the utilization rate ( 15 16 17 18 19 ). Early donor management can reduce post brain stem death hormonal and inflammatory changes and therefore improve donor organ function and increase the number of available lungs for transplantation.…”

“…Many ODM procedures that are meant to maintain optimal organ function have increased in complexity. This implies that the involvement of an experienced intensive care physician early in the donation process is crucial for the conversion of each and every donor . Another factor that could explain the success of the OPC is the conversion of profoundly marginal donors.…”
