ECCO2R as a bridge to a decision in type II respiratory failure

Abstract: Introduction. End-of-life decisions are often time consuming and difficult for everyone involved. In some of these cases extracorporeal life support systems could potentially be used not only as a bridge to treatment but as a tool to buy time to allow patient’s participation in decision making and to avoid further futile invasive procedures. “…Case reports have noted the use of ECCO 2 R as a bridge to redo lung transplantation 18 , as a bridge to decision for refractory ESLD in a patient who eventually refused lung transplantation 19 , and as a bridge to recovery in a lung transplant candidate who presented with AE-COPD in order to avoid intubation. 20 One of the largest case series describing the use of ECCO 2 R in lung transplantation bridging was reported by colleagues from the Medical University of Vienna where a total of 20 patients with refractory hypercapnic failure were bridged with either VV ECCO 2 R or AV ECCO 2 R. 7 Median duration of bridging time was 8 days, and 19/20 (95%) of patients were successfully bridged to transplantation.…”
