Economic Analysis in a Diagnosis Related Groups System for Two-stage Exchange of Prosthetic-joint Infections

Abstract: Background: There is a constant increase of joint arthroplasties performed, with an infectious risk of 1-2%. Different therapeutic options for prosthetic-joint infections exist, but surgery remains essential. With a two-stage exchange procedure, a success rate above 90% can be expected. Currently, there is no consensus regarding the optimal interval duration between explantation and reimplantation. This retrospective study aimed to assess the economic impact of a two-stage exchange from a single-hospital persp… Show more

“…In our hands, self-made hip and knee spacers are minimum 40-50% cheaper than prefabricated spacers and spacer molds. This is a major economic advantage in this already expensive surgery (15). In a DRG system, this difference cannot be neglected.…”

“…Die Erlössituation bei der Behandlung periprothetischer Hüftinfektionen ist aufgrund der unzureichenden Abbildung im bisherigen DRG-System nicht angemessen und stellt für die Kliniken eine große ökonomische Belastung dar [ 11 , 14 ], da die anfallenden hohen Behandlungskosten durch die Fallpauschalen des DRG-Systems nicht ausreichend gedeckt werden und dennoch von den Kliniken getragen werden müssen [ 2 , 15 , 17 , 27 ].…”

“…Periprosthetic joint infection (PJI) has an incidence of approximately 1 % after total hip arthroplasty (THA) and 2 % after total knee arthroplasty (TKA) in large register-based studies (Kurtz et al, 2010;Gundtoft et al, 2015;Huotari et al, 2015). PJI is associated with high morbidity and mortality (Kurtz et al, 2012;Webb et al, 2014;Lum et al, 2018;Natsuhara et al, 2019) and represents a high economic and logistic burden on the healthcare system (Kurtz et al, 2012;Vanhegan et al, 2012;Haddad et al, 2017;Fischbacher et al, 2018;Sousa et al, 2018;Schwartz et al, 2020).…”
