Effect of factor VIII concentrate on leucocyte cytokine production: characterization of TGF‐beta as an immunomodulatory component in plasma‐derived factor VIII concentrate

Abstract: Clinical and subclinical immunological abnormalities have been reported in HIV-seronegative haemophiliacs. The mechanisms by which these abnormalities arise remain unclear. As cytokines are important biological response modifiers, the effect of a FVIII concentrate on production of a range of cytokines, by a variety of cells, was investigated. A whole blood technique was used and the in vitro modulation of cytokine synthesis by an intermediate-purity plasma-derived factor VIII (pdFVIII) concentrate was analysed… Show more

“…Cytokine determination was performed as previously reported (12). Briefly, at 24 h, 100 l 20 mM EDTA/ phosphate-buffered saline (PBS) was added to one of the whole blood or PBMC culture tubes, which was vortexed vigorously for 20 s to remove adherent cells.…”

“…15 The detailed process whereby transfusions can induce immunosuppression is, however, still undefined and may include various mechanisms involving leukocyte-derived active mediators generated during blood storage, which may in turn impair the activity of immune cells. [16][17][18][19][20] In this context, we have recently shown that nonleukodepleted stored red blood cells (RBCs) and hemoderivatives contain high levels of immunoregulatory molecules, such as soluble human leukocyte antigen (HLA) class I and Fas ligand. 21,22 These molecules have been found to inhibit mixed lymphocyte responses and cytotoxic T-cell activity in allogeneic and in autologous combinations as well as to induce apoptosis in Fas ϩ cells.…”

“…Ruconest has the highest specific activity and purity, containing fewer nontherapeutic proteins compared with the plasma-derived C1-INH products. Whether these impurities (like the identified immunoglobulin heavy chain in Berinert) also have immunomodulatory effects, as described for similar intermediate purity type plasma-derived concentrates, remains to be determined [58,59].…”
